Frequency and prevalence of drug therapy related problems after hospitalization

Background: Drug-related problems (DRPs) are perceived to occur frequently when patients discharged from the hospital. (DRP) “an event or circumstance involving drug therapy that actually potentially interferes with desired health outcome”. Pharmacists influence care outcomes through identification and resolution of related (DRPs). 
 Aim objectives: The study is aimed assess frequency prevalence in after hospitalization Jayabharath Hospital, Nellore. To estimate opportunities for pharmacist provide pharmaceutical care. A probabilistic convenient sampling was done problems, those who aged above 18 years either sex were included. Pregnant patients, having a unwillingness participate excluded. Methodology: This cross sectional conducted between December 2020 May 2021 on home Nellore . Most our population within age group 30 45years (33.33 %) followed by 60 75years (25.00 %).among present most them married (83.3 illiterates (62.5 %).
 Results Discussion: Nearly 50 drugs responsible among Phenytoin found be leading relation development Where as Nifedipine least problem producing population.
