Hypercoagulopathy, acquired coagulation disorders and anticoagulation before, during and after extracorporeal membrane oxygenation in COVID-19: a case series

چکیده

Background: Thromboembolism and bleeding contribute to Coronavirus disease 2019 (COVID-19)’s morbidity mortality are also frequent complications of venovenous extracorporeal membrane oxygenation (vvECMO). As the interaction underlying pathologies caused by vvECMO in COVID-19 is barely understood, we designed this study better differentiate coagulation disorders patients before, during after vvECMO-support. Methods: Observational case series, six consecutive with acute respiratory distress syndrome supported treated anaesthesiologic ICU a third level University ECMO-centre. We measured routine parameters assessed factors. conducted advanced von Willebrand factor (VWF) multimer analysis, platelet aggregometry immunological screening. Results: identified various phases disorders: Initially, intensely activated highly increased VWF VIII activity COVID-19, then severe acquired dysfunction leading spontaneous finally, hypercoagulopathy explantation. Five developed abnormalities enhancing coagulation. Conclusions: Coronavirus-induced coagulopathy cannot be discriminated via ‘routine’ tests. Precise specific analyses followed appropriate treatment may help us develop tailored therapeutic concepts manage described above.

Endogenous glycosaminoglycan anticoagulation in extracorporeal membrane oxygenation

A heparin-like effect was recently described in infants, children, and adults receiving bivalirudin while supported on extracorporeal membrane oxygenation following cardiopulmonary bypass. This is most likely due to endogenous release of glycosaminoglycans from vascular endothelium and mast cells and is associated with longer duration of extracorporeal membrane oxygenation and an increased inci...

Extracorporeal Membrane Oxygenation in Drug Overdose: A Clinical Case Series

Overdose of cardiovascular medications such as beta blockers and calcium channel blockers cause impaired cardiac contractility, vasoplegia, and/or rhythm disturbances. In addition to conventional management of limiting absorption, increasing elimination and hemodynamic support intravenous (IV) calcium infusion, hyperinsulinemia-euglycemia therapy, glucagon infusion, and IV lipid emulsion have b...

Coagulation monitoring during extracorporeal membrane oxygenation: the role of thrombelastography.

Patients undergoing extracorporeal membrane oxygenation (ECMO) are at an increased risk for developing coagulopathies due to the adverse effects of extracorporeal circulation on the hemostatic mechanism. Methods of determining causative factors of bleeding diathesis are often inconsistent and non-specific. ECMO patients require aggressive transfusion therapy with autogenic blood products to sta...

Thromboelastography-based anticoagulation management during extracorporeal membrane oxygenation: a safety and feasibility pilot study

BACKGROUND There is no consensus on the management of anticoagulation during extracorporeal membrane oxygenation (ECMO). ECMO is currently burdened by a high rate of hemostatic complications, possibly associated with inadequate monitoring of heparin anticoagulation. This study aims to assess the safety and feasibility of an anticoagulation protocol for patients undergoing ECMO based on thromboe...
