Intravascular Lithotripsy for Peripheral Artery Calcification: Mid-term Outcomes From the Randomized Disrupt PAD III Trial

چکیده

BackgroundEndovascular treatment of calcified peripheral artery lesions may be associated with suboptimal vessel expansion, increased complication risk, and reduced long-term patency. The primary endpoint from the Disrupt PAD III randomized controlled trial (RCT) demonstrated superior procedural success in patients treated intravascular lithotripsy (IVL) vs percutaneous transluminal angioplasty (PTA). present study evaluates patency after 1 2 years this population.MethodsThe RCT enrolled 306 moderately-to-severely femoropopliteal arteries IVL (n = 153) or PTA prior to DCB stenting. powered secondary effectiveness was at year, defined as freedom clinically driven target lesion revascularization plus restenosis determined by duplex ultrasound. Acute failure requiring stent placement during index procedure prespecified a loss patency.ResultsPrimary year significantly greater arm (80.5% 68.0%, P .017). requirement for provisional stenting lower group (4.6% 18.3%, < .0001). Freedom (IVL: 95.7% PTA: 98.3%, .94) rates 90.0% 88.8%, .48) were similar between groups year. At years, remained (70.3% 51.3%, .003).ConclusionsThe 1-year achieved, confirming consistent safety followed facilitate durable approach heavily largely without requirement.

Mid-term results of the Radial Artery Patency and Clinical Outcomes randomized trial.

BACKGROUND The radial artery (RA), as an alternative to the saphenous vein or the right internal thoracic artery (RITA) for coronary artery bypass grafting, has gained considerable interest over the years. A randomized controlled trial was undertaken to assess the suitability of the radial artery as a conduit. METHOD The Radial Artery Patency and Clinical Outcomes (RAPCO) trial is a double-ar...

Buccal Fat Pad for Management of Cerebrospinal Fluid Leakage Using Endoscopy: A Randomized Clinical Trial

Background and Aim: Several surgical techniques have been so far used for treating Cerebrospinal Fluid (CSF) leakage (rhinorrhea) such as using abdominal fat or fascia lata, but these methods have complications such as the presence of several surgeons in variable fields in the operating room or cosmetic complications for donors such as the surgical scar. This study aimed to investigate using bu...

Peripheral Artery Disease (PAD) — Definition, Stages and Treatment

Peripheral artery disease, shortly referred to as PAD, is a condition caused due to atherosclerosis. Estimated to be affecting around 200 million people in 2010, the intermittent claudication is a medical entity of rising impact, which every prospective physician should master. The underlying atherosclerosis is not only the most common arterial vascular disease but also the pathologic substrate...
