چکیده
Abstract Background Cardiac magnetic resonance (CMR) and cardiopulmonary exercise testing (CPET) have provided important insights into the prevalence of early abnormalities in COVID-19 patients. It is currently unknown whether such persist over time relate to ongoing symptoms. Purpose To describe longitudinal trajectory on CMR CPET moderate severe patients assess their relationship with Methods Fifty-eight previously hospitalised 30 age, sex, body mass index, comorbidity-matched controls underwent CMR, a symptom-based questionnaire at 2–3 months (2–3m). Repeat assessments (including gas transfer) were performed 46 6 (6m). Results During admission, 1/3rd needed ventilation or intensive care (Table 1) three (5%) had raised troponin. On preserved left (LV) right ventricular (RV) volumes function 2–3m from infection. By 6m, LV did not change but RV end diastolic volume decreased (mean difference −4.3 mls/m2, p=0.005) increased +3.2%, p<0.001, Fig. 1A). Patients higher native T1 (a marker fibroinflammation) compared 1, 1B), which normalised by 6m. Extracellular was normal improved Native T2, myocardial oedema, differ between serial CMR. At 2–3m, late gadolinium enhancement (LGE) (p=0.023) became comparable 6m (p=0.62). Six (12%) LGE myocarditis pattern one (2%) infarction. None active using Modified Lake Louise Criteria. Lung imaging (T2-weighted) revealed parenchymal 2/3rds months. The extent 1). Gas transfer (DLco) worse those lung (77% vs 91% predicted, p=0.009). reduced peak oxygen consumption (pVO2) (80.5% 93.3% p=0.001) 1C). 49% submaximal tests (respiratory exchange ratio <1.1), reducing 25% (p=0.057). VE/VCO2 slope, efficiency, abnormal 1D). symptoms (chest pain, dyspnoea, palpitations, dizziness syncope) present 83% 52% (p<0.001). There no significant association parameters persistent cardiac (Fig. 1E). Conclusions Cardiopulmonary (on CPET) moderate-severe post Despite this, continued experience measured parameters.
