چکیده

Abstract Background Spontaneous coronary artery dissection (SCAD) is a non-atherosclerotic cause of myocardial infarction unclear aethiology. Histopathological analyses have identified increased inflammation in patients with SCAD [1]. Recently the peri-coronary adipose tissue attenuation (PCAT) derived from standard computed tomography angiography (CTA) has been established as reproducible method for measuring vascular which enhances risk stratification atherosclerosis [2]. Purpose To characterize pancoronary and vessel-specific by means CTA-derived PCAT recent compared individuals without prior SCAD. Methods Patients confirmed referred to tertiary center between 2017 2021 who underwent CTA routine cardiac evaluation were included this retrospective study. Subjects matched 1:1 no within same timespan, using scanner imaging protocol, arteries free obstructive disease similar clinical characteristics (Table 1). was analyzed on end-diastolic reconstructions semi-automated software methodology (Figure 1A) [3]. Proximal 40 mm all major vessels >2 diameter well SCAD-related vessel included. The primary outcome defined mean right (RCA), left anterior descending (LAD) circumflex (LCX) artery. Secondary RCA averaged LAD LCX (PCAT LCA). Results final cohort comprised 35 (median 6.1 mo. [4.0–16.8] since SCAD, 94.3% female) non-SCAD group. Pancoronary higher group (−80.1 [IQR: −74.5, −84.6] vs. −83.4 HU [−78.7, −88.0], P=0.008) suggestive inflammation. Vessel-specific (−80.7 [−70.4, −86.9] −85.3 [−80.4, −90.6] HU, P<0.001) borderline LCA (−79.2 [−73.9, −83.2] [−78.0, −88.3] P=0.056) respectively 1B). In previously dissected not significantly different unaffected (−78.4 [−73.7, −89.0] −80.9 [−74.9, −83.3], P=0.77; Figure 1C). While numerically decreased time association statistically significant when stratified into tertiles according 1D). Conclusions pericoronary suggesting perivascular inflammatory activity. This restricted vessel, nor reflected serum analysis persists over time. Funding Acknowledgement Type funding sources: None.

Can coronary computed tomography angiography replace invasive angiography? Coronary computed tomography angiography cannot replace invasive angiography.

O ne million patients undergo invasive coronary angiogra-phy (CA) in the United States every year. 1 In view of its invasive nature and related costs, noninvasive imaging of coronary imaging has the theoretical appeal to improve comfort for patients, reduce cost, and refine patient selection to limit invasive procedures to patients requiring treatment for ischemic coronary artery disease (CAD)....

[Non-invasive assessment of coronary artery bypass grafts by computed tomography: comparison with conventional coronary angiography].

INTRODUCTION AND OBJECTIVES Although it has been demonstrated that the diagnostic accuracy of computed tomography in the non-invasive assessment of major epicardial coronary arteries is high, only a few studies have evaluated the technique's reliability in assessing coronary artery bypass grafts. The aim of this study was to determine the diagnostic accuracy of multidetector computed tomography...

Computed tomography coronary angiography in patients with acute myocardial infarction and normal invasive coronary angiography

BACKGROUND Three to five percent of patients with acute myocardial infarction (AMI) have normal coronary arteries on invasive coronary angiography (ICA). The aim of this study was to assess the presence and characteristics of atherosclerotic plaques on computed tomography coronary angiography (CTCA) and describe the clinical characteristics of this group of patients. METHODS This was a multic...

Non-invasive coronary computed tomographic angiography for patients with suspected coronary artery disease: the Coronary Angiography by Computed Tomography with the Use of a Submillimeter resolution (CACTUS) trial.

BACKGROUND Non-invasive coronary angiography by multislice spiral computed tomography (MSCT) is a promising method for the diagnosis of coronary artery disease (CAD). However, the clinical role of this method has not been established for specific patient cohorts. Therefore, the objective of the current prospective, blinded study was to investigate the diagnostic value of coronary MSCT angiograp...

Non-invasive coronary angiography with high resolution multidetector-row computed tomography. Results in 102 patients.

AIMS A new generation of multidetector-row CT (MDCT) scanners allows complete coronary coverage using retrospective ECG gating and 1mm slices. The purpose of this study was to investigate the potential of high resolution MDCT angiography with retrospective gating for detection of coronary artery stenoses. METHODS AND RESULTS A total of 102 patients underwent both conventional and MDCT coronar...
