Non Selective his Bundle Pacing: As Physiological as S-HBP but Simpler and Faster

چکیده

We have read with interest Dr. Burri’s Electrocardiographic Analysis for His Bundle Pacing at Implantation and Follow up [1]. A comprehensive guide to understand electrocardiographic (ECG) nuances of (HBP) is presented in this review. The authors, outstanding didactics, describe a well-organized analysis challenging HBP characteristics confirm conduction tissue (selective or non-selective HBP) implant follow-up. Caught our interest, particular, the paragraph where practical pacing benefits (NSHBP) are emphasized. In context we would like point out statement that “ventricular myocardium adjacent atrioventricular septum, near bundle can also result NSHBP”. Above all, paramount clinical importance, when Burri et al. conclude “No significant differences cardiac mechanical synchrony outcome been found between SHBP Our group total agreement these essential features, having abandoned many years ago by developing very simple technique place standard leads para-Hisian area (Figure 1). Concordant “With (NS-HBP), lead usually positioned ventricle site (HB) surrounded proximity myocardial tissue”. That why named “para-Hisian pacing”. daily practice, same time, verify prove physiological capture system monitor online analyzing ECG signal variance [8] (Synchromax®) [2].

His Bundle Pacing: Techniques and Outcomes.

Traditional right ventricular pacing prolongs the electrical and mechanical activation of the heart, which reduces the pump function, and in selected patients may lead to pacing-induced heart failure. Pacing the His bundle preserves the normal or near-normal activation pattern of the ventricles that prevents the detrimental effects of right ventricular pacing. This review discusses the techniqu...

Acute His Bundle Injury during Permanent His Bundle Pacing: Mechanistic Insights into Intra- Hisian Block

Permanent His bundle pacing (HBP) is a physiological alternative to right ventricular pacing. Acute injury to the His bundle can occur during permanent HBP in the form of transient or permanent bundle branch blocks. Observations of mechanical injury to the His bundle during HBP lead fixation and electrophysiological response to HBP provides additional mechanistic insights into the pathology of ...

Making Neural QA as Simple as Possible but not Simpler

Recent development of large-scale question answering (QA) datasets triggered a substantial amount of research into end-toend neural architectures for QA. Increasingly complex systems have been conceived without comparison to simpler neural baseline systems that would justify their complexity. In this work, we propose a simple heuristic that guides the development of neural baseline systems for ...

The potential and reality of permanent his bundle pacing.

Right ventricular apex pacing can have deleterious effects. Our aims were to investigate how many patients referred for permanent pacing were suitable candidates for permanent His bundle pacing, and to determine the proportion in whom such pacing was successful. All cases of suprahisian block and most cases of infrahisian block (71.4%) were corrected by temporary His bundle pacing. However, per...
