P-137 DE GARENGEOT`S HERNIA: AN UNUSUAL PRESENTATION OF APPENDICITIS. CASE REPORT AND REVIEW OF THE LITERATURE

Abstract Introduction The presence of the appendix within a femoral hernia was first described by René De Garengeot in 1731. Less than 200 cases have been reported literature with an incidence 0.5–5% all hernias. Case Description A 69-year-old woman presented 72-hour history painful right-sided groin swelling. She denied fever or any gastrointestinal symptomatology. Clinical examination revealed non-reducible right mass. X-ray did not show bowel obstruction. Laboratory found increased leukocyte count (14.10 K/μL) 77% neutrophils. underwent emergency surgery. An inguinotomy performed, observing incarcerated ring. It showed clinical signs appendicitis no abscess perforation. Appendectomy performed. repaired polypropylene plug fixed to Cooper's and Gimbernat's ligaments. Discussion Garengeot`s affects women four times more compared men. most common symptoms are tenderness bulge (usually right-sided). diagnosis is usually performed intraoperatively, although CT-scan useful as complementary test. Due its low incidence, there still standardized recommendations on treatment this type hernia. approach open surgery via inguinal incision. Laparoscopic has too. Mesh use should be considered contaminated field. Conclusion entity We consider make early order prevent complications.
