P-295 Prognostic impact of clinico-pathologic parameters in early- and average-onset curatively resected colorectal cancer

Nearly a third of patients with curatively resected colorectal cancer experience recurrence. However, prognostic data do not discern sporadic early onset (EO-CC) from average (AO-CC). The aim this study was to estimate the incidence and relevance routinely assessed clinico-pathologic parameters across age cohorts. We compared comprehensive array characteristics in non-metastatic EO-CC (age 60) treated tertiary center between 2000-2020, using Fisher’s Exact Test. Logistic regressions Kaplan Meier curves were performed determine correlations patient outcomes. A total 216 surgically primary colon adenocarcinoma met criteria for inclusion (EO-CC: n=73, mean 42 years, 44.4% male gender; AO-CC: n=143, 71 52.5% gender). Compared AO-CC, group more likely have family history (52.4% vs 28.4%, p=0.002), present tumor obstruction (58.7% 24.8%, p 20 nodes removed, 71.4% 40.4%, < 0.001; adjuvant chemotherapy stage 2: 54.3% 22.9%, p=0.003, 3: 95.2% 75%, p=0.08). Overall survival longer (87.3% 78.4% log-rank p=0.001) as expected, however cancer-specific outcomes significantly inferior shorter relapse-free (76% 86%, p=0.002) 1.6 time recurrence during designated period. In both groups, 10% died cancer. group, all deaths cancer-related, most due active disease one capecitabine toxicity, while 47% related. Recurrence equally associated factors impact confirmed among AO-CC patients. had despite exhibiting identical histopathological features receiving aggressive surgical oncological treatment. Further research should elucidate causes manifestations potentially biology.
