P.171 Intradural-extramedullary spinal cavernoma with nerve root association: a case report and review of the literature

Background: Although 5% of cavernomas occur in the spine, intradural-extramedullary are exceptionally rare. We present one such case cavernoma associated exclusively with a nerve root, and review literature for similar lesions. Methods: Case Report. A 45-year old male patient presented six-month history numbness paresthesia affecting his lower extremities bilaterally. MRI demonstrated 16mm lesion at T3-4, compressing cord. The was surgically removed en-bloc, requiring root sacrifice. patient’s symptoms completely resolved post-operatively, he remained asymptomatic 3-month follow up. Results: Including case, 71 cases spinal have been reported, including 50 confirmed involvement. Patients most frequently between ages 40-59 (41%) lesions lumbar level (54%). Confirmed subarachnoid hemorrhage 14% presentation, although 23% acutely. 49% longstanding/progressive symptoms, 11% mixed acute-on-chronic presentation. 37% primarily radiculopathy, 21% myelopathy, pain. Good postoperative recovery documented 80% cases. Conclusions: Intradural-extramedullary may demonstrate propensity radiculopathy presentation exhibit substantial risk. Literature supports role surgical resection.
