P42 An audit to assess prescribing of paediatric CD TTOs

Aim To establish the root cause leading to an increased amount of time spent for a pharmacist triage paediatric controlled drug (CD) take out (TTO) prescription. Method Mixed methods were used collect data. Initially, raw data was collected over period three weeks identify any CD TTO’s that required intervention from pharmacist. We recorded if correct TTO prescription template being used, number prescriptions, clinically and legally how long it took complete by (ward or dispensary based). The not sufficient accurate picture problem. For this reason, we also prescribing tests. open book tests are completed all new doctors coming in rotation.
