چکیده
Background Imaging of the sacroiliac joints, especially with magnetic resonance imaging (MRI), is an important tool for early diagnosis axial spondyloarthritis (axSpA). Interpretation joint can vary according to readers’ experience, but it currently unknown if and how assessment differs between academic hospitals community based rheumatological care. Objectives To investigate (1) agreement local central reading images (X-ray MRI) from axSpA patients, (2) explore potential differences patients diagnosed in hospital compared centres. Methods The BelGian Inflammatory Arthritis spoNdylitis cohort (Be-GIANT) includes newly biological-naïve that fulfil ASAS classification criteria, at outpatient clinic eight centres Flanders. X-ray MRI joints (SIJ) enrolled November 2010 August 2020 were assessed by rheumatologist (‘local reading’) two calibrated readers (‘central definite radiographic sacroiliitis modified New York criteria (X-SIJ) active ASAS/OMERACT definition a positive (MRI-SIJ). Central resolved discrepant cases consensus. Inter-reader reliability was Cohen’s Kappa, % overall, negative agreement. Results Among 271 included (n=205 hospital, n=66 hospital), 231 X-SIJ 208 MRI-SIJ available (Table 1). disagreed on 30/231 (13%) (κ=0.44, moderate); 4/231 (1.7%) reclassified as 26/231 (11.3%) not showing sacroiliitis. Overall higher rheumatologists (90.5% vs. 70.7%, p<0.001). 53/208 (25.4%) (κ=0.36, fair); majority (51/208, 24.5%). agreed proportion versus (77.2% 63.4%, p=0.07). Conclusion In prevalence low. Sacroiliitis overcalled both non-academic settings, underscoring need continuous educational trainings. Table 1. Agreement centers. MRI and X-ray in axial spondyloarthritis: the relationship between inflammatory and structural changes
Demonstration of an association between inflammation and spinal ankylosis has been challenging. Until the advent of MRI, prospective study was not possible due to inaccessibility of tissue. Recent studies using MRI have described an association between the presence of bone edema at vertebral corners on MRI and the subsequent development of syndesmophytes at the corresponding vertebral corners o...
متن کاملAnkylosing Spondylitis and Axial Spondyloarthritis.
Chronic back pain is common worldwide and is cared for by a variety of providers, but specific, satisfactory treatment is often lacking. Ankylosing spondylitis, an inflammatory disorder that in its extreme form can lead to the bony fusion of vertebral joints, is an uncommon but well-established cause of chronic back pain. During the past decade, ankylosing spondylitis has come to be considered ...
متن کاملImmunohistological analysis of active sacroiliitis in patients with axial spondyloarthritis
The sacroiliac joints (SIJs) are one of the most common sites involved in axial spondyloarthritis (axSpA), and there are few studies on the histopathology of the SIJ in this group of patients.Mononuclear cell infiltrates in the bone marrow and fibrous tissue resembling a pannus formation were the pathological features of early sacroiliitis in our previous study. We undertook a further immunohis...
متن کاملMight axial myofascial properties and biomechanical mechanisms be relevant to ankylosing spondylitis and axial spondyloarthritis?
Ankylosing spondylitis and axial spondyloarthropathy have characteristic age- and sex-specific onset patterns, typical entheseal lesions, and marked heritability, but the integrative mechanisms causing the pathophysiological and structural alterations remain largely undefined. Myofascial tissues are integrated in the body into webs and networks which permit transmission of passive and active te...
متن کاملThe Nonradiographic Axial Spondyloarthritis, the Radiographic Axial Spondyloarthritis, and Ankylosing Spondylitis: The Tangled Skein of Rheumatology
Since 1984 the diagnosis of ankylosing spondylitis (AS) has been based upon the modified New York (mNY) criteria with mandatory presence of radiographic sacroiliitis, without which the diagnosis is not tenable. However, it may take years or decades for radiographic sacroiliitis to develop delaying the diagnosis for long periods. It did not matter in the past because no effective treatment was a...
