چکیده

Background Nintedanib (NIN) is an antifibrotic drug used to slow the progression of systemic sclerosis-related interstitial lung disease (SSc-ILD), reducing decline in forced vital capacity (FVC). High-resolution chest tomography (HRCT) gold standard imaging technique assess SSc-ILD, whereas ultrasound (LUS) a useful and repeatable non-invasive tool which increasingly clinical practice. Objectives The aim this work was investigate usefulness LUS during follow-up SSc-ILD patients treated with NIN. Methods Adult SSc starting NIN for ILD were enrolled followed 12 months. performed at baseline quarterly on 55 antero-posterior spaces, evaluating pleural line irregularities (PLI; semi-quantitative score 0-1-2) number B-lines (BL), according OMERACT definition. FVC investigated every six months, HRCT following patient-reported outcomes administered quarterly: Leicester cough questionnaire St.George respiratory questionnaire. During follow-up, all aforementioned investigations assessed stability, improvement, or worsening from baseline, then concordance variations. Results Ten (70% female, mean age 62±10.9 years) enrolled. One patient had prematurely discontinue due unbearable gastrointestinal adverse effects. other 9 achieving stabilization 87.5% cases. PLI BL both showed declining trend over time (Figure 1), 12-months significant overall reduction (p=0.02 PLI; p<0.01 BL). At final evaluation, improved 55.5% cases stable rest, resulted worsened 62.5% rest. these two techniques very low (12.5%). quite good (61.5% 46.1% Moreover, variations greater (69.2% 42.3%, respectively) than (35.7% 28.5%, respectively). Conclusion We Changes fairly closely reflected functional subjective patients. correlation questionnaires. seems progress regardless efficacy function, therefore could be monitoring receiving Figure 1. The Assessment of Health-Related Quality of Life in Scleroderma-Interstitial Lung Disease

Introduction: Pulmonary involvement is the most common cause of mortality and disability in patients with systemic sclerosis and it significantly affects the quality of life in these patients. Therefore, early diagnosis and treatment of pulmonary involvement seems necessary in patients with SSc. In this study, we aimed to assess the health-related quality of life (HRQoL) in patients with Sclero...

Difficulties in diagnosis of systemic sclerosis-related interstitial lung disease

We discuss a case that demonstrates the difficulties in making a confident diagnosis of interstitial lung disease (ILD) associated with systemic sclerosis (SSc). Atypical symptoms can be present, suggesting the diagnosis of an underlying connective tissue disease (CTD), but it can take several years before a definite diagnosis can be made because there are insufficient features to fulfill the p...

Genetic Susceptibility to Interstitial Lung Disease Associated with Systemic Sclerosis

Systemic sclerosis (SSc) is a connective tissue disease that is characterized by tissue fibrosis, microvasculopathy, and autoimmunity. Interstitial lung disease (ILD) is a common complication of SSc and is one of the frequent causes of mortality in SSc. Although the exact etiology of SSc remains unknown, clinical and experimental investigations have suggested that genetic and environmental fact...

- Cerinic Interstitial lung disease in systemic sclerosis

Lung involvement frequently complicates systemic sclerosis (SSc), provoking loss of quality of life and a poor expectation of survival. For this reason an early diagnosis of lung involvement is warranted: high-resolution computed tomography (HRCT), pulmonary function tests (PFT), lung scintigraphy with DTPA and bronchoalveolar lavage (BAL) are mandatory to define and follow-up pulmonary interst...

Interstitial lung disease in systemic sclerosis

Systemic sclerosis (SSc) is a connective tissue disease characterised by autoimmune features, widespread vasculopathy and fibrosis of the skin and internal organs. The pattern of internal organ involvement and the natural history of the disease are highly variable. Although the etiology is unknown, most studies point to links between genetic predisposition and environmental factors. SSc has bee...
