Renal Biomarker combinations predict early CRRT need in a mixed ICU population

چکیده
Background: Acute kidney injury (AKI) is associated with markedly increased morbidity and mortality in critically ill patients often necessitates the use of continuous renal replacement therapy (CRRT). The aim present study was to compare predictive performance urine neutrophil gelatinase (uNGAL), plasma cystatin C (pCysC), serum creatinine (sCr), their combinations for CRRT requirement within first 7 days post-admission a general ICU.Method material: A total 81 consecutive ICU were included analysis. AKI defined according AKIN criteria. Biomarkers’ abilities evaluated by area under receiver operating characteristics (AUC-ROC) curves. Results Conclusions: occurred 21% 14% them needed post-admission. two novel biomarkers, as well sCr had moderate requirement. most efficient (sCr+ uNGAL) (sCr+uNGAL+pCysC) better AUC-ROCs (0.845 0.84, respectively) than that any individual biomarker (sCr, pCysC, uNGAL, 0.81, 0.74 0.80, respectively). Renal need one week post admission compared each alone.

Early mortality on continuous renal replacement therapy (CRRT): the prairie CRRT study
BACKGROUND Patients with acute kidney injury (AKI) requiring renal replacement therapy (RRT) have an increased short-term and long-term risk of mortality. In most North American intensive care units (ICUs), these patients receive continuous renal replacement therapy (CRRT). OBJECTIVE We aim to identify clinical and demographic factors associated with mortality within 24 h of initiating CRRT. ...
متن کاملExperience of Plasmapheresis Performed with Crrt Monitors by Icu Staff
Results A total of 49 sessions were performed in 5 patients with neurological disease. 80% of our patients were female. Median age: 42 (IQR 35-71) years. Median number of sessions in each cycle: 4 (range 3-6). Plasma volume exchange ratio was 1:1. Replacement fluid used was Hemosol B0 TM (Gambro) with addition of albumin (to achieve a concentration of 3-4 g/dL) and KCl (4.1 ± 0.3 mmol/L). Sessi...
متن کاملRenal replacement therapy III: IHD, CRRT, SLED.
Acute renal failure in critically ill patients is a growing clinical problem. Options for renal replacement therapy in these patients use convective and diffusive clearance and may be intermittent, as in classic hemodialysis, or continuous. New ways of delivering dialysis in the intensive care unit, such as sustained low-efficiency dialysis, are also under development. It may be that renal repl...
متن کاملRisk prediction of ICU readmission in a mixed surgical and medical population
BACKGROUND Readmission to intensive care units (ICU) is accompanied with longer ICU stay as well as higher ICU, in-hospital and 30-day mortality. Different scoring systems have been used in order to predict and reduce readmission rates. METHODS The purpose of this study was to evaluate the Stability and Workload Index for Transfer (SWIFT) score as a predictor of readmission. Further, we wante...
متن کاملTIMP-1 as a biomarker in the ICU
In the previous issue of Critical Care, the Spanish group of Lorente and colleagues reports results in which the role of the 372 T/C polymorphism of tissue inhibitor of metalloproteinase (TIMP)-1 (rs 4898) and serum levels of TIMP-1 were related to the outcome of 275 patients with severe sepsis [1]. Th ey found the T-allele in the 372 T/C genetic poly morphism of TIMP-1 and increased levels of ...
