Successful enucleation of a large sciatic schwannoma with intraoperative neurostimulation-guided dissection: a case report

Advances in intraoperative neurophysiological monitoring (IONM) have enabled safer peripheral nerve tumor surgery. We report a patient whom large sciatic schwannoma was successfully removed using stimulation. A 62-year-old male presented with palpable mass the right posterior upper thigh, just below gluteal fold. Magnetic resonance imaging showed an approximately 9-cm encapsulated solid middle of nerve. After vertical skin incision laterally curved end extending to inferior region, proximal secured biceps femoris and distal medial muscle. avoided causing trauma viable fascicles by dissecting through “safe zone,” as confirmed muscle contractions induced stimulation on surface. In this report, we present details our surgical approach portion use IONM for safe excision.
