چکیده

Aims Use of sodium–glucose co-transporter 2 inhibitors (SGLT2i) in real-world heart failure (HF) is poorly characterised. In contemporary patients with HF and type diabetes mellitus (T2DM) we assessed over time SGLT2i use, clinical characteristics outcomes associated use. Methods results Type enrolled the Swedish Registry between 2016–2018 were considered. We performed multivariable logistic regression models to assess independent predictors use Cox a 1:3 propensity score-matched cohort relevant subgroups investigate association outcomes. Of 6805 eligible T2DM, 376 (5.5%) received SGLT2i, whose increased 12% on treatment at end 2018. Independent younger age, specialty care, ischaemic disease, preserved kidney function, absence anaemia. Over median follow-up 256 days, was 30% lower risk cardiovascular (CV) death/first hospitalisation (hazard ratio 0.70, 95% confidence interval 0.52–0.95), which consistent regardless ejection fraction, background metformin function. also all-cause CV death, hospitalisation, death/myocardial infarction/stroke. Conclusion time, more common specialist renal mortality morbidity.

Mechanisms of Heart Failure in Type 2 Diabetes Mellitus.

cardiovascular diseases. Over the last decade, we have significantly contributed to the development of important concepts in the field of insulin signal transduction and the molecular basis of regulation of gene expression and energy metabolism. Genetic studies in worm C. elegans indicated that the insulin/IGF-1 signaling pathway via PI-3kinase→protein kinase B/Akt→Forkhead transcription factor...

Thiazolidinediones and the risk of incident congestive heart failure among patients with type 2 diabetes mellitus.

BACKGROUND Clinical trials suggest that thiazolidinediones (TZDs) may increase the risk of congestive heart failure (CHF). However, their effect on the risk of incident CHF in unselected populations has not been thoroughly investigated. METHODS Using data from the UK's General Practice Research Database, we conducted a case-control study within a population-based cohort of patients with type ...

Sodium-Glucose Cotransporter 2 Inhibitors for Heart Failure

In this issue (P94), Kutoh reports type 2 diabetic patients who showed changes in cardiac function which is concomitant with changes in ketone bodies by using one of sodium glucose cotransporter 2 inhibitors (SGLT2i), tofogliflozin [1], proposing a possible mechanism for SGLT2i-mediated prevention of heart failure. EMPA-REG OUTCOME trial showed a striking relative risk reduction in hospitalizat...

Association of depression with evolution of heart failure in patients with type 2 diabetes mellitus

BACKGROUND Depression is a prevalent, independent predictor of mortality in patients with heart failure (HF). Depression is also common in type 2 diabetes mellitus (T2DM), which is itself an important risk factor for HF. However, association of depression with incident HF in T2DM is undefined. The aim of the present study was to evaluate the predictive value of depression in predicting incident...

Body mass index and heart failure among patients with type 2 diabetes mellitus.

BACKGROUND Epidemiological data on the association between body mass index (BMI) and heart failure (HF) risk among diabetic patients are rare. METHODS AND RESULTS We performed a prospective cohort study of risk for HF among 31 155 patients with type 2 diabetes mellitus (11 468 men and 19 687 women). Cox proportional hazards regression models were used to estimate the association of different ...
