Progress Report on Chiorambucil Therapy in Postsplenectomy Patients With Progressive Hairy Cell Leukemia
نویسنده
چکیده
H AIRY CELL leukemia, a disease characterized by splenomegaly without adenopathy, by pancytopenia, and by circulating malignant cells with prominent cytoplasmic projections requires therapy in most cases.”2 The manifestations of hairy cell leukemia result from a balance between (I) leukemic infiltration of the red pulp of the spleen by hairy cells, which leads to hypersplenism,3 and (2) leukemic infiltration of the bone marrow, which leads to decreased production of bone marrow elements and to peripheral blood cytopenia(s). In most cases, the hypersplenism can be corrected by splenectomy;4 however, approximately one-third of the patients have progressive disease in the bone marrow, which manifests itself either by a leukemic phase with various associated cytopenias or by progressive pancytopenia. In four earlier patients, we originally suggested treatment with chlorambucil for these manifestations of progressive disease.5 Five additional patients have subsequently been treated with chlorambucil. All of these nine patients are the subjects of this report.

Progress report on chlorambucil therapy in postsplenectomy patients with progressive hairy cell leukemia.
Of eight patients with progressive hairy cell leukemia who were treated with daily doses of an alkylating agent (chlorambucil, 4 mg) for at least 6 mo, seven have had an objective response, as measured by improved blood counts. Two patients were pancytopenic and had almost total bone marrow replacement with hairy cells at the initiation of chemotherapy; approximatley 6 mo later, their blood cou...
متن کاملTreatment of hairy cell leukemia (leukemic reticuloendotheliosis) II. Chlorambucil therapy in postsplenectomy patients with progressive disease.
Four postsplenectomy patients with progressive hairy cell leukemia were treated with daily low doses of an alkylating agent (chlorambucil, 4 mg). An objective response, measured by improvement in blood counts in four patients as well as decreased bone marrow involvement on core biopsy in three patients, could be documented within 6 mo. It is important to identify the postsplenectomy patient wit...
متن کاملA Case of Hairy Cell Leukemia Associated with Hepatitis B Surface (HBs) Antigenemia and Concurrent Hepatitis C Virus (HCV) Seropositivity
Literature review shows that hepatitis C infection may increase the risk of non-hodgkin lymphoma. Reactivation of hepatitis B infection has been reported in patients with hairy cell leukemia after chemotherapy and/or splenectomy. We present hepatitis B surface (HBs) antigenemia and concurrent hepatitis C virus (HCV) seropositivity in a case of hairy cell leukemia. Key Words: Hairy cell leukemi...
متن کاملResponse to splenectomy in 65 patients with hairy cell leukemia: an evaluation of spleen weight and bone marrow involvement.
Sixty-five patients with hairy cell leukemia underwent splenectomy; 27 had a complete remission as defined by a return in WBC, RBC, and platelet counts to a defined level, and 38 had a partial remission with a return of only one or two of these parameters to the defined level. The 5-yr actuarial survival for all patients is 68%; for CR patients it is 76%, and for PR patients 62%. The response t...
متن کاملClinical staging system for hairy-cell leukemia.
To find a clinical staging system for patients with hairy cell leukemia, 391 patients contributed by 22 centers were analyzed using the proportional hazard survival model. Attention was paid to nonsplenectomized patients to find a staging system to predict the survival length at the time of diagnosis. On the basis of hemoglobin level and spleen size at the time of diagnosis, 3 stages could be d...
