Endoscopic ultrasound-guided repositioning of a migrated metal hepatogastrostomy stent using foreign body forceps.

چکیده
Endoscopic ultrasound (EUS)-guided hepaticogastrostomy with a fully covered metal stent is an option for malignant biliary obstruction after a failed endoscopic retrograde cholangiopancreatography (ERCP) [1–3]; however, migration of the stent can be a fatal complication [3, 4]. We report a case in which a migrated stent was successful reset using a foreign body forceps. A 73-year-old woman developed abdominal pain and fever on the third day after EUS-guided hepaticogastrostomy with a metal stent (WallFlex Biliary RX, fully covered stent system; Boston Scientific, Galway, Ireland). A computed tomography (CT) scan showed that stent migration had occurred (●" Fig.1a, ●" Fig.1b, ●" Fig.1c) and an abdominal fluid collection had developed in the omental bursa (●" Fig.1d). Fig.1 Computed tomography (CT) scan images showing: a, b, c the migrated stent; d an abdominal fluid collection within the omental bursa.

choledochoduodenostomy (EUS-CDS) is reported with increasing frequency [1]. While stent migration into the bile duct is a possible complication of EUS-CDS, it has not previously been reported. We describe a case of complete stent migration into the bile duct, where the stent was successfully retrieved by inserting an endoscope directly into the bile duct after balloon dilation of the duodenal f...

The Hot AXIOS system is a new device for transgastric or transduodenal endoscopic drainage of a pancreatic pseudocyst or the gallbladder [1,2] using a lumen-apposing metal stent (LAMS) mounted on an electrocautery-enhanced introduction system. The device seems to be relatively safe in expert hands; however, the literature on management of its complications is limited [1–4]. Hereby, we report on...

Figure 1 The endoscope is inserted through the distal end of Several techniques for managing migrated and misplaced self-expanding esophageal metal stents have been described.1--5 However, most of these techniques apply only when there is distal stent dislodgment. When there is proximal stent migration or misplacement, and unless there is a lasso at the distal flange, trying to reposition the s...

A 51-year-old man was referred to our facility for endoscopic ultrasound-guided drainage of walled-off pancreatic necrosis. The cyst was first punctured with a 19-gauge needle (▶Fig. 1). A NAGI stent (Taewoong Medical, Seoul, South Korea) was then deployed under ultrasonographic and fluoroscopic guidance without difficulty (▶Fig. 2, ▶Video1). A computed tomography (CT) scan 6 weeks later reveal...
