Early maturity onset diabetes mellitus presenting as episodes of hypoglycaemia.

چکیده
Diabetes mellitus is a genetically determined disorder of metabolism which in it's fully developed clinial expression is characterized by fasting hyperglycaemia. In early maturity onset diabetes the most common abnormality is delayed secretion of insulin following a glycaemic stimulus which may then be followed by an exaggerated response (Seltzer et al., 1967). This disturbance of insulin in early diabetes can lead to hypogly-caemia which in many patients is not severe to be symptomatic. However, in an occasional patient symptoms related to hypoglycaemia are predominant, which cause diagnostic confusion and delay in appropriate treatment. In this case report we describe a patient with early maturity onset diabetes who presented with episodic symptoms of palpitation, sweating, weakness, visual blurring, tachycardia and lack of concentration which were from time to time ascribed to anxiety state and neurasthenia. The diagnosis of hypoglycaemia of early maturity onset diabetes was finally established by doing a five hour glucose tolerance test supported by insulin studies.

HNF4A mutation: switch from hyperinsulinaemic hypoglycaemia to maturity-onset diabetes of the young, and incretin response
BACKGROUND Hepatocyte nuclear factor 4α (HNF4A) is a member of the nuclear receptor family of ligand-activated transcription factors. HNF4A mutations cause hyperinsulinaemic hypoglycaemia in early life and maturity-onset diabetes of the young. Regular screening of HNF4A mutation carriers using the oral glucose tolerance test has been recommended to diagnose diabetes mellitus at an early stage. ...
متن کاملPersistent Hyperinsulinaemic Hypoglycaemia and Maturity Onset Diabetes of the Young (MODY) due to Heterozygous HNF4A Mutations
Objective: Mutations in the human HNF4A gene encoding the hepatocyte nuclear factor 4 alpha (HNF-4α) are known to cause maturity-onset diabetes of the young (MODY), which is characterized by autosomal dominant inheritance and impaired glucose-stimulated insulin secretion from pancreatic ß-cells. HNF-4α has a key role in regulating the multiple transcriptional factor networks in the islet. Recen...
متن کاملAssociation between diabetes, severe hypoglycaemia, and electroencephalographic abnormalities.
Serial electroencephalographic recordings were made in 70 diabetic children and findings were related to age at electroencephalography and at diagnosis, duration of diabetes, daily insulin dose, long term metabolic control assessed by glycated haemoglobin A1 (HbA1) concentrations, and severe hypoglycaemic episodes. Abnormalities were found in 18 (26%) of diabetic children, and in only five (7%)...
