Multivessel Coronary Artery Disease. Surgical Treatment versus Angioplasty

چکیده

The current technical step forwards in metallic stents (MS), bare metal stents (BMS) and drug-eluting stents (DES) respectively led interventional cardiologists to a rude therapeutic attitude, that even the most complex coronary lesions are being treated with angioplasty plus stents (TCA). About a third of all the patients with multivessel disease (MVD) in Europe are being treated with TCA instead of revascularization surgery (MRS), although the guidelines suggest this one as a superior treatment. Cost-effectiveness of bypass surgery versus stenting in patients with multivessel coronary artery disease.

PURPOSE To compare the cost-effectiveness of surgical and angioplasty-based coronary artery revascularization techniques, in particular, angioplasty with primary stenting. METHODS We used data from the Study of Economics and Quality of Life, a substudy of the Bypass Angioplasty Revascularization Investigation (BARI), to measure the outcomes and costs of angioplasty and bypass surgery in patie...

Percutaneous transluminal coronary angioplasty versus medical treatment for non-acute coronary heart disease: meta-analysis of randomised controlled trials.

OBJECTIVE To determine whether percutaneous transluminal coronary angioplasty (angioplasty) is superior to medical treatment in non-acute coronary artery disease. DESIGN Meta-analysis of randomised controlled trials. SETTING Randomised controlled trials conducted worldwide and published between 1979 and 1998. PARTICIPANTS 953 patients treated with angioplasty and 951 with medical treatmen...

Choosing between percutaneous coronary intervention and coronary artery bypass grafting for patients with multivessel disease: what can we learn from the Arterial Revascularization Therapy Study (ARTS)?

The ARTS trial was one of 11 randomized trials comparing an initial strategy of percutaneous coronary intervention (PCI) versus coronary artery bypass grafting (CABG) in the treatment of patients with multivessel coronary artery disease.1–11 Six of these trials used balloon angioplasty in the PCI arm,1–6 whereas the 5 most recent trials (of which ARTS was the largest, with 1205 patients) used s...

Contemporary Percutaneous Coronary Intervention Versus Balloon Angioplasty for Multivessel Coronary Artery Disease

Background—This investigation compares the results of contemporary percutaneous coronary intervention (PCI) with standard balloon angioplasty among patients with multivessel coronary disease. Patients having balloon angioplasty in the Bypass Angioplasty Revascularization Investigation (BARI) and those within the Dynamic Registry meeting BARI eligibility criteria were studied. Methods and Result...
