Levodopa-induced Dyskinesias and Their Management N Epidemiology

چکیده
Following a period of stable response to dopaminergic medication (the “honeymoon” period), Parkinson’s disease (PD) patients gradually develop two progressive clinical phenomena requiring changes in the clinical management: motor fluctuations and dyskinesias. These two phenomena are intrinsically interconnected, but often require opposite treatment attitudes, because increases of dopaminergic treatment often improve fluctuations, but worsen dyskinesias. A possible solution to this dilemma resides in the administration of additional anti-dyskinetic medication; unfortunately, however, drugs with enough anti-dyskinetic potency have not yet been developed.

Glutamate NMDA receptor dysregulation in Parkinson's disease with dyskinesias.
Levodopa-induced dyskinesias are a common complication of long-term therapy in Parkinson's disease. Although both pre- and post-synaptic mechanisms seem to be implicated in their development, the precise physiopathology of these disabling involuntary movements remains to be fully elucidated. Abnormalities in glutamate transmission (over expression and phosphorylation of N-methyl-D-aspartate rec...

Levodopa-induced dyskinesia is a major complication of long-term dopamine replacement therapy for Parkinson's disease that becomes increasingly problematic in advanced Parkinson's disease. Although the cause of levodopa-induced dyskinesias is still unclear, recent work in animal models of the corticostriatal system has suggested that levodopa-induced dyskinesias might result from abnormal contr...
متن کاملThe acute brain response to levodopa heralds dyskinesias in Parkinson disease
OBJECTIVE In Parkinson disease (PD), long-term treatment with the dopamine precursor levodopa gradually induces involuntary "dyskinesia" movements. The neural mechanisms underlying the emergence of levodopa-induced dyskinesias in vivo are still poorly understood. Here, we applied functional magnetic resonance imaging (fMRI) to map the emergence of peak-of-dose dyskinesias in patients with PD. ...
متن کاملClinical Aspects and Management of Levodopa-Induced Dyskinesia
In Parkinson's disease, one of the most troublesome dilemmas is the treatment of levodopa-induced dyskinesia. After a few years, chronic treatment with levodopa is associated with the development of dyskinesias. Strategies to delay or to reduce dyskinesias are based on the change of levodopa dosing or the early use of dopamine agonists. Dopamine agonists with different pharmacological profile a...

OBJECTIVES To determine whether continuous waking day dopaminergic stimulation with the dopamine agonist apomorphine can reduce levodopa induced dyskinesias in Parkinson's disease METHODS 19 patients with severe unpredictable refractory motor fluctuations and functionally disabling levodopa induced dyskinesias were treated with continuous subcutaneous apomorphine monotherapy for a minimum dur...
