چکیده

That means there is considerable overlap in what we would consider the true euthyroid state in a patient. Hence, a population may be well characterised by this screening process, but it doesn't closely reflect the individual situation. As a result, an abnormal result of the population-based screening should not necessarily translate into an automatic decision on the requirement of treatment. The view on treatment consequences can therefore not readily be derived from the screening in such a way as to assume every patient with an abnormal screening test will be treated. That assumption is by no means justified.

Are lower TSH cutoffs in neonatal screening for congenital hypothyroidism warranted?

When newborn screening (NBS) for congenital hypothyroidism (CH) using thyroid-stimulating hormone (TSH) as a primary screening test was introduced, typical TSH screening cutoffs were 20-50 U/L of whole blood. Over the years, lowering of TSH cutoffs has contributed to an increased prevalence of detected CH. However, a consensus on the benefit deriving from lowering TSH cutoffs at screening is la...

Congenital hypothyroidism with delayed TSH elevation in low-birth-weight infants: incidence, diagnosis and management.

OBJECTIVE To evaluate the incidence of congenital hypothyroidism (CH) with delayed TSH elevation among low-birth-weight (LBW) newborns in North-Eastern Italy and to verify if they need a second or third screening. DESIGN Analysis of clinical and biochemical data of newborns affected by CH with delayed TSH elevation identified by neonatal screening. METHODS Data of all newborns with birth we...

Prediction of congenital hypothyroidism based on initial screening thyroid-stimulating-hormone

BACKGROUND In thyroid-stimulating-hormone (TSH)-based newborn congenital hypothyroidism (CH) screening programs, the optimal screening-TSH cutoff level is critical to ensuring that true cases of CH are not missed. Screening-TSH results can also be used to predict the likelihood of CH and guide appropriate clinical management. The purpose of this study is to evaluate the predictive value of vari...

Thyroid Stimulating Hormone Assay as a Screening Tool of Thyroid Disease

Serum thyroid stimulating hormone (TSH) estimation is an ideal investigation for thyroid disease. It gives most comprehensive information about whether a patient has a thyroid disease and what is the cause. It has been frequently observed in clinical settings that complete profile of thyroid function tests is advised for screening and vague symptoms. Aims and objectives of current study were to...
