Switching from branded to generic antiepileptic drugs as a confounding factor and unpredictable diagnostic pitfall in epilepsy management.

چکیده
Patients with epilepsy are frequently invited (often by their own GP) to switch from branded to generic anti-epileptic drugs. The main reason for this change in treatment is that generic drugs cost less, and this has important implications in health expenditure control. Despite the “essential similarity” of drugs, the prescription of generic products may, however, expose patients to additional and, in some cases, unpre dictable risks (Crawford et al. 1996, Argumosa and Herranz 2005).

Compulsory generic switching of antiepileptic drugs: high switchback rates to branded compounds compared with other drug classes.
PURPOSE Compulsory generic substitution of antiepileptic drugs (AEDs) may lead to adverse effects in epilepsy patients because of seizure recurrence or increased toxicity. The study objectives were (a) to quantify and compare the switchback rates from generic to brand-name AEDs versus non-AEDs, and (b) to assess clinical implications of switching from branded Lamictal to generic lamotrigine (LT...
متن کاملAntiepileptic drugs: the drawbacks of generic substitution.
The need to curb the rising costs of health care has led policy-makers in many countries to consider widespread generic substitution of brand-name drugs as an eff ective means to cut costs. For example, on January 5, 2010, the NHS in England began a 3-month consultation to seek views on plans to automatically substitute branded drugs with generic versions in primary care. However, doctors, pati...
متن کاملSubstitution of anticonvulsant drugs
Changing from branded drugs to generic alternatives, or between different generic formulations, is common practice aiming at reducing health care costs. It has been suggested that antiepileptic drugs (AEDs) should be exempt from substitution because of the potential negative consequences of adverse events and breakthrough seizures. Controlled data are lacking on the risk of substitution. Howeve...
متن کاملEconomic burden associated with the use of generic antiepileptic drugs in the United States.
This study quantifies the economic burden associated with generic-versus-branded use of antiepileptic drugs (AEDs) in the United States. Adult patients with epilepsy receiving carbamazepine, gabapentin, phenytoin, primidone, or zonisamide were selected from the PharMetrics database. By use of an open-cohort design, patients were classified into mutually exclusive periods of generic-versus-brand...
متن کاملUnderstanding patients' perspective in the use of generic antiepileptic drugs: compelling lessons for physicians to improve physician/patient communication
BACKGROUND Epilepsy is a condition in which consistency of treatment is paramount to successful management and for most patients, effective seizure control can be achieved. Given the severe consequences of even a single breakthrough seizure, patients should be afforded every opportunity to succeed on their given regimens. DISCUSSION Some experts argue that global policy on generic antiepilept...
