Arterial stiffness, systolic blood pressure, and logical treatment of arterial hypertension.

چکیده

Arterial stiffening is the principal cause of increasing systolic pressure with advancing years and in patients with arterial hypertension. It is associated with progressive arterial dilation and is due to degeneration of the arterial wall, probably as a consequence of repetitive cyclic stress; it increases systolic pressure directly by increasing amplitude of the pressure wave generated by a given flow impulse from the heart and indirectly by increasing wave velocity so that wave reflection from the periphery occurs earlier, augmenting pressure in late systole. The first mechanism affects pressure in both the central and peripheral arteries, the second predominantly in the central arteries. Change in brachial systolic pressure with age underestimates the rise in systolic pressure in the aorta and left ventricle. Arterial stiffness is reduced passively with reduction in arterial pressure. Drugs have little or no direct effect on arterial stiffness but can markedly reduce wave reflection. In patients with stiffened arteries, reduction in wave reflection decreases aortic systolic pressure augmentation. The decreased systolic pressure in central arteries brought about by this mechanism is not detected when systolic pressure is measured in a peripheral (brachial or radial) artery but can be inferred from change in contour of the pressure wave recorded in peripheral arteries.

Presentation of a Non-invasive Method of Estimating Arterial Stiffness by Modeling Blood Flow and Arterial Wall Based on the Determination of Elastic Module of Arterial Wall

Introduction: Arterial stiffness is an important predictor of cardiovascular risk. Several indices have been  introduced to estimate the arterial stiffness based on the changes in the brachial blood pressure. Since the  substitution  of  the  blood  pressure  changes  in  the  central  arteries  such  as  carotid  with  the  blood  pressure  changes in the brachial results in error in the blood...

‌بررسی اثر عصاره هیدروالکلی برگ زیتون (Olea europea L.) بر روی فشار‌خون و تداخل اثر آن با سیستم آدرنرژیک در موش صحرایی نر

Introduction: Hypertension is one of the most common and important disease factors imposed by the modern lifestyle, in particular by reduced physical activity and an unbalanced lipid-rich diet. One of the best methods for treatment of hypertension is the use of medical herbs that have moderate effects on blood pressure such as Olive (Olea europea L.). Therefore, the purpose of this study was to...

Arterial stiffness as underlying mechanism of disagreement between an oscillometric blood pressure monitor and a sphygmomanometer.

Oscillometric blood pressure devices tend to overestimate systolic blood pressure and underestimate diastolic blood pressure compared with sphygmomanometers. Recent studies indicate that discrepancies in performance between these devices may differ between healthy and diabetic subjects. Arterial stiffness in diabetics could be the underlying factor explaining these differences. We studied diffe...

Blood Vessels Noninvasive Assessment of Arterial Stiffness Should Discriminate Between Systolic and Diastolic Pressure Ranges

Arterial stiffening plays an important role in the development of hypertension and cardiovascular diseases. The intrinsically nonlinear (ie, pressure-dependent) elastic behavior of arteries may have serious consequences for the accuracy and interpretation of arterial stiffness measurements and, ultimately, for individual patient management. We determined aortic pressure and common carotid arter...

Hypertension Highlights Arterial Stiffness and Hypertension Emerging Concepts

Arterial stiffness is increasingly recognized as an important prognostic index and potential therapeutic target in patients with hypertension. It is closely linked to, but by no means synonymous with, raised blood pressure, and its physiopathology is still not fully understood. Aortic stiffness and arterial pulse wave reflections are key determinants of elevated central systolic pressure and ar...
