PLATELETS AND THROMBOPOIESIS Defective regulatory B-cell compartment in patients with immune thrombocytopenia

چکیده
B lymphocytes producing antiplatelet autoantibodies play a major role in autoimmune thrombocytopenia (ITP). However, certain B cells, including the human CD19 CD24hiCD38hi subpopulation, possess regulatory functions mediated partly by IL-10. In a cohort of chronic ITP patients with low platelet counts who consisted of patients off treatment, we found a lower frequency of CD19 CD24hiCD38hi in the peripheral compartment of nonsplenectomized patients (P .03). IL-10 expression after activation was decreased in all ITP circulating CD19 subpopulations (P < .03), and inhibition of monocyte TNFexpression by activated B cells was reduced in patients with platelet numbers of < 50 109 cells/L (P .001), indicating that regulatory B cells of patients with ITP are functionally impaired in their ability to dampen monocyte activation. Interestingly, in nonsplenectomized patients whose platelet counts were elevated after treatment with thrombopoietic agents, the frequency of CD19 CD24hiCD38hi B cells was increased compared with those before treatment (P .02). Altogether, these data indicate a compromised regulatory B-cell compartment as an additional defect in immune regulation in patients with chronic ITP that may be restored in responders to thrombopoietic treatment. Immune thrombocytopenia (ITP) is an autoantibody-mediated bleeding disorder with both accelerated platelet destruction and impaired platelet production. We and others have described impaired regulatory CD4 CD25hi T cells (Treg) numbers and/or suppressive function in ITP patients. Clinical trials using thrombopoietic agents to stimulate platelet production have shown favorable outcomes in ITP pa...

Elevated level of B-cell activating factor (BAFF) has been implicated in the pathogenesis of some autoimmune diseases. Blockade of receptor and ligand binding by decoy receptor has demonstrated a clinical benefit in both oncologic and immunologic diseases. In this report, we have detected plasma BAFF and BAFF mRNA expression in immune thrombocytopenia (ITP) patients by enzyme-linked immunosorbe...
متن کاملPLATELETS AND THROMBOPOIESIS A murine model of severe immune thrombocytopenia is induced by antibody- and CD8 T cell–mediated responses that are differentially sensitive to therapy
Immune thrombocytopenia (ITP) is a bleeding disorder characterized by antibodyopsonized platelets being prematurely destroyed in the spleen, although some patients with ITP may have a cell-mediated form of thrombocytopenia. Although several animal models of ITP have been developed, few mimic primary chronic ITP nor have any shown cell-mediated platelet destruction. To create this type of model,...

Immune thrombocytopenic purpura (ITP) is a common autoimmune disorder resulting in isolated thrombocytopenia. It is a bleeding disorder characterized by low platelet counts due to decreased platelet production as well as increased platelet destruction by autoimmune mechanisms. ITP can present either alone (primary) or in the setting of other conditions (secondary) such as infections or altered ...
