Understanding skeletal muscle wasting in critically ill patients

چکیده
Methods Objective: The objective was to perform a comprehensive prospective characterization of skeletal muscle wasting, defining the pathogenic roles of altered protein synthesis and breakdown. Design: Prospective observational study. Setting: Patients were recruited from a university and community hospital in England. Subjects: The study involved 63 critically ill patients >18 years old who were anticipated to be intubated >48 hours and to spend >7 days in the ICU and to survive their ICU stay. Subjects were enrolled within 24 hours of admission. Outcomes: Muscle loss was determined through serial ultrasound measurement of the rectus femoris crosssectional area (CSA) on days 1, 3, 7, and 10. In a subset of patients, the fiber CSA area was quantified along with the ratio of protein to DNA on days 1 and 7. Histopathological analysis was performed. In addition, muscle

Protein metabolism and gene expression in skeletal muscle of critically ill patients with sepsis.
Muscle wasting negatively affects morbidity and mortality in critically ill patients. This progressive wasting is accompanied by, in general, a normal muscle PS (protein synthesis) rate. In the present study, we investigated whether muscle protein degradation is increased in critically ill patients with sepsis and which proteolytic enzyme systems are involved in this degradation. Eight patients...
متن کاملNeuromuscular electrical stimulation for preventing skeletal-muscle weakness and wasting in critically ill patients: a systematic review
BACKGROUND Neuromuscular electrical stimulation (NMES) therapy may be useful in early musculoskeletal rehabilitation during acute critical illness. The objective of this systematic review was to evaluate the effectiveness of NMES for preventing skeletal-muscle weakness and wasting in critically ill patients, in comparison with usual care. METHODS We searched PubMed, CENTRAL, CINAHL, Web of Sc...
متن کاملIntensive care unit—acquired weakness (ICUAW) and muscle wasting in critically ill patients with severe sepsis and septic shock
Sepsis presents a major health care problem and remains one of the leading causes of death within the intensive care unit (ICU). Therapeutic approaches against severe sepsis and septic shock focus on early identification. Adequate source control, administration of antibiotics, preload optimization by fluid resuscitation and further hemodynamic stabilisation using vasopressors whenever appropria...
متن کاملTitle efficacy of phosphodiesterase 5 inhibitor on distant burn-induced muscle autophagy, microcirculation, and survival rate.
Skeletal muscle wasting is an exacerbating factor in the prognosis of critically ill patients. Using a systemic burn injury model in mice, we have established a role of autophagy in the resulting muscle wasting that is distant from the burn trauma. We provide evidence that burn injury increases the autophagy turnover in the distal skeletal muscle by conventional postmortem tissue analyses and b...
متن کاملAcute skeletal muscle wasting in critical illness.
IMPORTANCE Survivors of critical illness demonstrate skeletal muscle wasting with associated functional impairment. OBJECTIVE To perform a comprehensive prospective characterization of skeletal muscle wasting, defining the pathogenic roles of altered protein synthesis and breakdown. DESIGN, SETTING, AND PARTICIPANTS Sixty-three critically ill patients (59% male; mean age: 54.7 years [95% CI...
