Varón de 68 años, con antecedente de adenocarcinoma esofágico (ypT3N1M0) tratado con quimioterapia neoadyuvante y esofagectomı́a segú n técnica de Ivor Lewis mı́nimamente invasiva (tiempo abdominal) hace un año. Acudió a Urgencias por dolor abdominal, vómitos y estreñimiento de 3 dı́as de evolución. A la exploración presentaba disnea, el abdomen era doloroso, sobre todo en hemiabdomen derecho, con defensa y signos de irritación peritoneal. En la radiografı́a de tórax se observó una burbuja aérea supradiafragmática (fig. 1), por lo que se realizó tomografı́a computarizada que mostró ascenso del colon transverso a través del hiato diafragmático, con distensión de colon derecho y colapso del colon izquierdo (fig. 2). c i r e s p . 2 0 1 4 ; x x ( x x ) : x x x – x x x
