Failure of right ventricular recovery of fallot patients after pulmonary valve replacement: delay of reoperation or surgical technique?

چکیده
underwent monitoring during both the initial treatment and study drug discontinuation periods (n 5 163), ischemia was less frequently observed during both monitoring periods in the enoxaparin group (18.4% vs. 32.2%, p 5 0.045 and 25% vs. 46%, p 5 0.005, respectively). Further, the time to first ischemic episode was significantly earlier among UFH-treated patients, consistent with a superior early antithrombotic effect of enoxaparin and a reduction in the composite clinical end point of death, myocardial infarction (MI), and need for urgent revascularization (3). Therefore, we believe that enoxaparin has been shown to be superior to UFH based upon the consistent and statistically significant reductions in the composite and double (death/MI) end points in ESSENCE and TIMI 11B (4). In contrast, “extending the duration and slower weaning” of UFH is an unproven method of administration, and, in fact, the cost-savings realized with enoxaparin ultimately make it the less expensive option (5).

Right ventricular Hemodynamic Alteration after Pulmonary Valve Replacement in Children with Congenital Heart Disease
Introduction: In patients who underwent surgery to repair Tetralogy of Fallot, right ventricular dilation from pulmonary regurgitation may be result in right ventricular failure, arrhythmias and cardiac arrest. Hence, pulmonary valve replacement may be necessary to reduce right ventricular volume overload. The aim of present study was to assess the effects of pulmonary valve replacement on rig...

Background and Objective: The aim of this study was to analyze our indications, surgical procedures, and clinical outcomes of patients undergoing reoperation after surgical correction of tetralogy of Fallot (TOF). Methods: Thirty seven consecutive patients who underwent reoperation late after intra-cardiac repair of TOF within a period of 10 years were assessed. Results: The most co...
متن کاملOutcomes of Pulmonary Valve Replacement for Correction Pulmonary Insufficiency after Primary Repair of Tetralogy of Fallot (TOF)
Background Total correction of Tetralogy of Fallot (TOF) anomaly in early childhood has been practiced in many centers with good results, but in some of patients after few years sever Pulmonary valve insufficiency occurred. Materials and Methods At a cross- sectional study from January 2015 to January 2016, 10 patients who had history of primary repair of TOF with free pulmonary insufficiency (...

Background: It has not been yet developed a ost suitable method to treat right ventricular failure due to pulmonar artery hypertention resulted from ventricular arrhythia. Though some case reports about patients ready to heart transplant using left assist device or biventral assist device as bridge have been published in Journals, but nocase concerning application of centrifugal pump for treatm...
متن کاملSurgical Management of Chronic Pulmonary Regurgitation After Relief of Right Ventricular Outflow Tract Obstruction
Relief of right ventricular (RV) outflow tract obstruction in tetralogy of Fallot or similar physiology often results in pulmonary regurgitation (PR). The resultant chronic volume overload can lead to RV dilatation, biventricular dysfunction, heart failure symptoms, arrhythmias and sudden death. Although pulmonary valve replacement (PVR) can lead to improvement in the functional class and a sub...
متن کاملThe role of cardiac magnetic resonance to address the treatment of choice for pulmonary valve replacement late after repair of Tetralogy of Fallot
BACKGROUND Pulmonary valve incompetence is usually well tolerated after tetralogy of Fallot repair but may result in late progressive right heart failure as manifested by increasing fatigue, dyspnea, and frequently arrhythmias. METHODS All patients who underwent pulmonary valve replacement in our center late after repair of tetralogy of Fallot were reviewed. RESULTS Eighty-five patients had...
