Pränatale Kortisontherapie eines Giant Fetal Hepatic Hemangiomas

چکیده
Infantile hepatische Hämangiome sind benigne vaskuläre Tumoren und nach Hepatoblastomen die zweithäufigsten Lebertumoren [1–3]. Die Hepatomegalie bildet sich nach initial raschem Wachstum bis zum Alter von 12–18 Monaten vollständig zurück [4]. Eine Behandlung ist erforderlich, um ernsten Komplikationen, wie zum Beispiel die thrombozytopenische kongestive Herzinsuffizienz, die thrombozytopenische Verbrauchskoagulopathie (Kasabach-Merritt-Syndrom) oder auch eine Tumorrupturblutung aufgrund von arteriovenösen Shunts, vorzubeugen. Die neonatale Mortalitätsrate liegt bei 70–90 % [5]. Die Behandlung fetaler Hämangioendotheliome wird in der Literatur kontrovers diskutiert [6–9]. Prednison-Dosierungen zwischen 3 und10 mg/kg/Tag werden seit vielen Jahren zur Behandlung infantiler Hämangiome eingesetzt, mit einer Erfolgsrate von 50– 80 % [10,11]. Bisher sind nur wenige Fallberichte zur pränatalen Kortisontherapie in der Literatur publiziert worden [8, 12, 13].
