Hemodynamic effects of orally administered isosorbide dinitrate in patients with congestive heart failure.

چکیده
The efficacy of orally administered isosorbide dinitrate (IS) has been questioned. Recently vasodilators have been shown to lower left ventricular filling pressure (LVFP) and raise cardiac output in heart failure. In the present study oral IS was evaluated in 12 patients with high LVFP due to heart failure. After right heart catheterization, patients with LVFP > 14 mm Hg were randomized double-blind to receive either 20 mg IS

Hydralazine and isosorbide dinitrate: comparative central and regional hemodynamic effects when administered alone or in combination.
Central and regional hemodynamic studies were performed before and after the oral administration of 75 mg of hydralazine and/or 20 mg of isosorbide dinitrate in 15 patients with low-output congestive heart failure. Hydralazine increased mean cardiac output 17-25%, mean renal blood flow 19%, and limb blood flow 17% (all p < 0.05). Mean hepatic blood flow did not change significantly with hydrala...
متن کاملVasodilator therapy for chronic left ventricular failure.
The effects of chronic oral vasodilator therapy were studied in a group of patients with refractory congestive heart failure. Fifteen patients were treated acutely with intravenous sodium nitroprusside and sublingual isosorbide dinitrate. After continuous therapy with nitroprusside and isosorbide dinitrate for up to 72 hours the patients were then placed on isosorbide dinitrate and oral phenoxy...
متن کاملImproved exercise capacity and differing arterial and venous tolerance during chronic isosorbide dinitrate therapy for congestive heart failure.
We studied 30 patients with moderate-to-severe congestive heart failure in a double-blind, randomized, placebo-controlled trial to determine the acute and long-term effects of isosorbide dinitrate on clinical status and on resting and exercise hemodynamics. Seventeen patients received placebo and 13 isosorbide dinitrate. First-dose isosorbide dinitrate (40 mg orally) decreased resting and exerc...
متن کاملHaemodynamic effects of oral salbutamol alone and in combination with sublingual isosorbide dinitrate in patients with severe congestive cardiac failure.
To examine possible augmentation of the effects of isosorbide dinitrate by salbutamol, haemodynamic measurements were made in 10 patients with severe chronic congestive cardiac failure who received isosorbide dinitrate 2.5 to 25 mg sublingually and salbutamol 4 to 12 mg orally, alone and in combination. Isosorbide dinitrate reduced mean left ventricular filling pressure from 29 to 18 mmHg and i...
متن کاملIntravenous isosorbide dinitrate in patients with refractory pump failure and acute myocardial infarction.
We studied the hemodynamic effects of isosorbide dinitrate administered by continuous i.v. infusion to 22 patients with chronic refractory pump failure and 18 with pump failure due to acute myocardial infarction. In patients with severe pump failure, i.v. ISDN markedly decreased pulmonary capillary wedge pressure (p less than 0.001), moderately increased cardiac output (p less than 0.01), and d...
