چکیده

was obtained at operation or necropsy from 22 patients, including 14 with obstructive and eight with nonobstructive ASH, and studied by both light and electron microscopy. Many hypertrophied, bizarrely shaped, and abnormally arranged cardiac muscle cells, presumably a morphologic manifestation of the genetically transmitted myocardial defect in ASH, were present in the ventricular septum of all patients. In patients with obstructive ASH, these abnormalities were either absent or rarely found in muscle from the left and right ventricular free walls. This observation suggests that functional limitation in these patients is due largely to left ventricular outflow obstruction. In contrast, numerous disorganized cells were extensively distributed in the left and right ventricular feee walls from seven of eight symptomatic patients with nonobstructive ASH, suggesting that these abnormalities probably contribute importantly to functional impairment in such patients.

Differences in Distribution of Myocardial Abnormalities in Patients with Obstructive and Nonobstructive Asymmetric

Patients with typical idiopathic hypertrophic subaortic stenosis (IHSS) represent only one subgroup of a cardiac disease in which the characteristic anatomic abnormality is asymmetric septal hypertrophy (ASH). In most patients with ASH, left ventricular outflow obstruction is absent and cardiac dysfunction presumably is due to widespread involvement of the left ventricle by an underlying myocar...

Biventricular cineangiography in the evaluation of patients with asymmetric septal hypertrophy.

Echocardiographic studies have demonstrated that the characteristic anatomic defect in idiopathic hypertrophic subaortic stenosis (IHSS) is asymmetric septal hypertrophy (ASH). Although echocardiographic diagnosis of ASH is extremely sensitive, occasionally adequate echocardiograms cannot be obtained. Moreover, echocardiographic assessment of the extent of the underlying cardiomyopathic process...

Hypertrophic cardiomyopathy in Friedreich's ataxia: symmetric or asymmetric?

We evaluated 15 patients with Friedreich's ataxia (FA) to define the incidence of myocardial involvement and the type of cardiomyopathy observed. All patients with FA had either ECG, vectocardiographic or echocardiographic abnormalities, suggesting some degree of myocardial involvement. In contrast to reports indicating that asymmetric septal hypertrophy (ASH), often obstructive, is associated ...

Hypertrophic obstructive cardiomyopathy. Assessment by echocardiographic and Doppler ultrasound techniques.

Hypertrophic obstructive cardiomyopathy is a disease of the myocardium that can be assessed echocardiographic and transcutaneous Doppler ultrasound techniques. Four patients are presented with various patterns of the disease, and the frequently familial incidence is illustrated. The importance of ultrasonic evidence for asymmetric septal hypertrophy in all stages is emphasized and evidence of r...
