چکیده

A 35-year-old female nurse presented with an 8-month history of headaches often precipitated by valsalva type maneuvers. The patient occasionally experienced tingling in her face and left arm associated with several episodes of blurred vision. The neurologic exam was normal as was the electroencephalogram. Plain skull films showed asymmetric pneumatization of sphenoid sinus (Fig. 1A). Computed tomography (CT) scan in the axial plane showed a large extra-axial lesion of the left hemisphere (Fig. 1 B) showing mass effect. Magnetic resonance (MR) imaging in the coronal imaging plane at Tl -weighting (800/20/1) showed a large arachnoid cyst of the left anterior and middle cranial fossa and ballooning of the adjacent sphenoid sinus into the lesion (Fig. 1 C). A far smaller arachnoid cyst of the right temporal lobe tip was also noted. An arachnoid cyst of the left hemisphere was confirmed surgically at the time of placement of cystoperitoneal shunt.

OBJECTIVE To report 3 cases of pneumosinus dilatans of the sphenoid sinus associated with visual loss. MATERIALS AND METHODS Retrospective case series describing history of visual loss, visual examination, visual field deficits, and radiologic imaging. RESULTS Three patients developed visual loss associated with pneumosinus dilatans of the sphenoid sinus. CONCLUSIONS Pneumosinus dilatans ...

Spontaneous pneumocephalus caused by the association of pneumosinus dilatans and meningioma. Case illustration.

Pneumosinus dilatans (PSD) refers to an abnormally enlarged, air-filled paranasal sinus without radiological evidence of localized bone destruction, hyperostosis, or mucous membrane thickening. This disorder has been associated with meningioma, especially optic nerve sheath meningioma, but its pathophysiology remains unclear. Spontaneous pneumocephalus is an exceptional presentation of PSD and,...

Complex Frontal Pneumosinus Dilatans Associated with Meningioma: A Report of Two Cases and Associated Literature Review

Introduction  Pneumosinus dilatans (PSD) is a rare phenomenon involving the expansion of the paranasal sinuses, without bony destruction or a mass. Previously documented cases have demonstrated simple expansion of a solitary air cell. We present two unique cases of PSD in the presence of meningioma, in which complex new cells developed within the frontal sinus. One of the two patients developed...

Spontaneous drainage of an ethmoidal mucocele: a possible cause of pneumosinus dilatans.

Idiopathic expansion of a paranasal sinus containing air and without an associated mass has been termed pneumosinus dilatans [1] . This rare disorder has been known to occur in all the paranasal sinuses, but it most commonly affects the frontal sinus. Clinical symptoms include pain , visual loss, proptosis, galactorrhea, and hypopituitarism [2-4]. Proposed mechanisms of origin include developme...

Pneumosinus dilatans frontalis: a case report.

Deformity of the forehead is usually the first clinical presentation of the abnormally large frontal sinus caused by pneumosinus dilatans. We report the diagnosis and surgical treatment of a case. The type of osteotomy we describe is recommended as a method of surgical treatment for this condition when the aesthetic problem is the major complaint.
