Letter by Yang and Bai Regarding Article, “Safety of Intravenous Thrombolysis in Stroke Mimics: Prospective 5-Year Study and Comprehensive Meta-Analysis” To the Editor: We read with interest a recent article by Tsivgoulis et al on the safety of intravenous thrombolysis (IVT) in stroke mimics (SM). Through both their own institutional data and meta-analysis of 8 published series, the authors found a low risk of symptomatic intracranial hemorrhage among SMs receiving recombinant tissue-type plasminogen activator (tPA). Because tPA does not adversely affect the favorable natural history of SMs, the authors argued that delaying IVT to rule out a stroke mimic is not warranted in terms of safety. The decision to treat a patient with IVT for acute ischemic stroke (AIS) is often based on the patient’s medical history, clinical presentation, laboratory test results, and computed tomographic (CT) findings because of the limited time window thrombolysis is feasible for AIS. As a consequence, there will always be some patients treated with tPA who have no ischemic stroke but another medical condition mimicking a stroke. Because tPA has been proven safe in these patients, can we accept SM as unavoidable when managing suspected AIS? The answer is no because tPA is an unnecessary, inefficient, and potentially harmful treatment. It is also expensive, especially for patients who have to pay out of their own pockets. In light of this, we would like to ask the following questions:

Response to letter regarding article, "Safety of intravenous thrombolysis in stroke mimics: prospective 5-year study and comprehensive meta-analysis".
BACKGROUND AND PURPOSE Shortening door-to-needle time may lead to inadvertent intravenous thrombolysis (IVT) administration in stroke mimics (SMs). We sought to determine the safety of IVT in SMs using prospective, single-center data and by conducting a comprehensive meta-analysis of reported case-series. METHODS We prospectively analyzed consecutive IVT-treated patients during a 5-year perio...
متن کاملLetter by Radecki regarding article, "safety of thrombolysis in stroke mimics: results from a multicenter cohort study".
BACKGROUND AND PURPOSE Intravenous thrombolysis for acute ischemic stroke is beneficial within 4.5 hours of symptom onset, but the effect rapidly decreases over time, necessitating quick diagnostic in-hospital work-up. Initial time strain occasionally results in treatment of patients with an alternate diagnosis (stroke mimics). We investigated whether intravenous thrombolysis is safe in these p...
متن کاملSafety and feasibility of intravenous thrombolytic therapy in Iranian patients with acute ischemic stroke
Background: Thrombolytic therapy is the only approved treatment for acute cerebral ischemia. The hemorrhagic transformation is the greatest complication of this treatment, which may occur after recanalization of occluded artery. The aim of this study was to determine factors associated with clinical improvement and worsening in patients with acute ischemic stroke treated with intravenous th...
متن کاملThrombolysis in Stroke Patients; Problems and Limitations
Thrombolysis for stroke is being used in some developing countries. This study was designed to evaluate the problems of thrombolysis therapy in Iran. During January-July 2008, all patients with ischemic stroke admitted to Ghaem Hospital, Mashhad, northeast Iran, were enrolled in a prospective observational study. Ghaem Hospital is a tertiary care hospital that includes infrastructure for thromb...
متن کاملEfficacy and Safety of Endovascular Treatment versus Intravenous Thrombolysis for Acute Ischemic Stroke: A Meta-Analysis of Randomized Controlled Trials
BACKGROUND AND PURPOSE Although endovascular therapy (ET) is increasingly used in patients with moderate to severe acute ischemic stroke, its efficacy and safety remains controversial. We performed a meta-analysis aiming to compare the benefits and safety of endovascular treatment and intravenous thrombolysis in the treatment of acute ischemic stroke. METHODS We systematically searched PubMed...
