Brief Communication Lithium and Triiodothyronine Augmentation of Antidepressants

چکیده
There are now sufficient data to suggest that lithium is an effective augmentation strategy for patients with major depression who have an inadequate response to all classes of antidepressants (1). A recent metaanalysis supports the efficacy of T3 in treatment-resistant depression (2); these studies involving T3 were in patients taking tricyclic antidepressants, although a recent study supports the efficacy of T3 in SSRI-resistant depression (3). Further, there is some clinical evidence that lithium and T3, which both work as an augmentation strategy in up to 50% of cases (1,2), may be effective in different subgroups of depression patients (4,5). We reasoned that combined lithium and T3 might produce greater improvement in depression scores than either alone and, therefore, carried out a direct comparison of lithium and T3 alone and a combination, using a 2-week, double-blind, placebocontrolled design.

Triiodothyronine augmentation for the treatment of depression in substance misusers unresponsive to tricyclic antidepressants.
We report on two substance misusers with depression resistant to tricyclic antidepressant treatment who responded to triiodothyronine augmentation. The management of resistant depression, augmentation strategies with particular reference to triiodothyronine, and the possible mechanism of action of triiodothyronine are discussed.
متن کاملLithium's emerging role in the treatment of refractory major depressive episodes: augmentation of antidepressants.
BACKGROUND The late onset of therapeutic response and a relatively large proportion of nonresponders to antidepressants remain major concerns in clinical practice. Therefore, there is a critical need for effective medication strategies that augment treatment with antidepressants. METHODS To review the available evidence on the use of lithium as an augmentation strategy to treat depressive epi...
متن کاملHypothalamic-pituitary-thyroid system activity during lithium augmentation therapy in patients with unipolar major depression.
OBJECTIVE Lithium augmentation is an established strategy in the treatment of refractory depression, but little is known about predictors of response and its mode of action. There is increasing evidence that low thyroid function indices within the normal range are associated with a poorer treatment response to antidepressants, but previous studies on the hypothalamic-pituitary-thyroid (HPT) sys...
متن کاملLithium augmentation of nortriptyline for subjects resistant to multiple antidepressants.
Lithium augmentation, the most studied augmentation strategy for depression, has not been evaluated in patients with a history of non-response to multiple antidepressants. The objective of this study was to assess the efficacy of lithium augmentation for patients with a history of treatment resistant depression who also failed a prospective trial of nortriptyline. We enrolled 92 subjects with t...
متن کاملGhrelin Serum Concentrations Are Associated with Treatment Response During Lithium Augmentation of Antidepressants
Background Lithium augmentation of antidepressants is an effective strategy in treatment-resistant depression. The proteohormone ghrelin is thought to be involved in the pathophysiology of depression. The purpose of this study was to investigate the association of treatment response with the course of ghrelin levels during lithium augmentation. Method Ghrelin serum concentrations and severity...
