Editorial : Tube Feeding Old People

چکیده

The process of administering nutritious sustenance through a tube into the gut while avoiding the mouth and swallowing mechanisms is a nice example of collaboration or conflict between health care professionals arising from their medical, ethical and legal concerns. Two recent articles in the Journal' have drawn attention to these concerns and it is timely to analyse them further. When ethical and related legal issues arise in health care, consideration of the underlying medical aspects often clarifies, if not resolves, the dilemmas.

Substitute decision-making for cognitively impaired older people.

Editorials Éditoriaux A bout 10% of elderly residents in chronic care facilities are tube-fed. Yet there is persuasive evidence that common beliefs about tube-feeding cannot be supported: tube-feeding is not similar to spoon-feeding, it is not necessarily indicated in patients with aspiration pneumonia , swallowing evaluations are not very helpful in selecting patients for tube-feeding, and whe...

The Role of Percutaneous Endoscopic Transgastric Jejunostomy in the Management of Enteral Tube Feeding

Feeding-related adverse events after percutaneous endoscopic gastrostomy (PEG) such as aspiration pneumonia can impede the use of PEG. Percutaneous endoscopic transgastric jejunostomy (PEG-J) using large-bore jejunal tubes with gastric decompression function may improve outcomes by circumventing gastric passage during enteral nutrition and improving drainage of excessive gastric secretions. Thi...

Maintaining Enteral Nutrition in the Severely Ill using a Newly Developed Nasojejunal Feeding Tube with Gastric Decompression Function

For nutritional support of critically ill patients, the enteral route is preferred over the parenteral route. Although nasojejunal feeding can be superior to gastric feeding when gastrointestinal symptoms occur, it does not necessarily solve the problem of large gastric residual volumes. We report the successful use of a newly developed nasojejunal feeding tube with gastric decompression functi...

Spontaneous antegrade enteral migration of feeding jejunostomy tube.

Spontaneous migration of enteral feeding tubes is unusual. We report an 18-year-old man with corrosive stricture of the upper esophagus in whom feeding jejunostomy was performed using an 18F Levine's tube. Thirteen months later, he presented with absence of the tube. The patient was tolerating liquid diet and was managed conservatively. Serial radiographs and colonoscopy showed a steady progres...
