چکیده

Objectives Clinically amyopathic dermatomyositis (CADM) is considered as the subtype of dermatomyositis with amyopathy or hypomyopathy. Rapidly progressive interstitial lung disease (RP-ILD) is treatment-resistant and life-threatening in patients with CADM. Since useful markers for early diagnosis are necessary for early treatment in CADM patients with RP-IPD, this study aimed to investigate the differences of clinical and laboratory characteristics between alive patients (group A) and patients who died (group D). Methods Eleven patients with CADM complicating with RP-ILD were enrolled. Clinical manifestations and laboratory data before treatment were compared between group A and group D. Results Among them, six were alive and five died after treatment for RP-ILD. Serum IL-6 levels in group D were significantly higher than those in group A (median 19.7 vs 7.15 pg/ml; P=0.008). The stepwise regression analysis showed that IL-6 was the only prognostic factor significantly (p=0.035). Kaplan-Meier estimates showed that patient survivals were better in patients with IL-6 < 9pg/ml than those with > 9pg/ml (p=0.039). Other laboratory data were not different statistically between two groups. Conclusions Serum IL-6 may predict the prognosis of patients with CADM having Akita University

Amyopathic Dermatomyositis Associated with Histopathological Findings of Organizing Pneumonia and Pulmonary Vasculitis

BACKGROUND Clinically, amyopathic dermatomyositis is a clinically distinct subgroup of dermatomyositis characterised by unique dermatological manifestations without muscle involvement. Clinically, amyopathic dermatomyositis is frequently associated with interstitial lung disease, which usually has a rapidly progressive, fatal clinical course. Although clinically, amyopathic dermatomyositis-rela...

Non-Hodgkin lymphoma in a 13-year-old boy with provisional hypomyopathic juvenile Dermatomyositis

Provisional hypomyopathic juvenile dermatomyositis is a subgroup of clinically amyopathic juvenile dermatomyositis provisional. The diagnostic criteria include: Classic dermatomyositis skin lesions – which have to be confirmed by biopsy –, no involvement of proximal muscles, subclinical involvement of these muscles and normal level of muscle enzymes. These criteria should be present for a minim...

How Often are Pediatric Patients with Clinically Amyopathic Dermatomyositis Truly Amyopathic?

BACKGROUND Pediatric patients can present with skin manifestations of dermatomyositis without overt weakness (clinically amyopathic juvenile dermatomyositis [JDM]), but it is unclear how often this happens and how often they have subclinical muscle inflammation. OBJECTIVE Our goal was to determine the frequency of clinically amyopathic JDM and the frequency with which a thorough evaluation un...

Rapidly progressive interstitial pneumonia associated with clinically amyopathic dermatomyositis successfully treated with polymyxin B-immobilized fiber column hemoperfusion.

Amyopathic dermatomyositis (ADM) is a clinical subtype of dermatomyositis, characterized by the absence of motor weakness and the presence of normal muscle enzyme levels. ADM is sometimes accompanied by interstitial pneumonia that shows a rapid progressive course associated with a poor prognosis. We describe a 70-year-old man who presented rapidly progressive interstitial pneumonia associated w...
