Dr. M.R. Ogborn, Izaak Walton Killam Hospital for Children, PO Box 3070, Halifax, N.S. B3J 3G9 (Canada) Dear Sir, The recent excellent review of posttransplantation hypertension by Waltzer et al. [1] suggests that deterioration of renal function with captopril therapy may be indicative of the severity of transplant artery stenosis. We report a case in which labile renal dysfunction was precipitated by the use of captopril, in the presence of a narrowing of the transplant artery that was of dubious functional significance. mm Hg) developed, which was controlled with metoprolol 25 mg b.i.d. Captopril renography and duplex Doppler sonography in the diagnosis of renovascular hypertension.

BACKGROUND The purpose of this prospective study was to determine the clinical usefulness of captopril renal scintigraphy and duplex Doppler sonography in detecting haemodynamically significant renal artery stenosis (> or = 60%) and predicting cure or improvement of hypertension following revascularisation. METHODS Twenty-eight patients with moderate or high index of clinical suspicion of ren...

Reversible renal failure due to captopril in a patient with transplant artery stenosis. Case report.

A patient who was subsequently shown to have stenosis of the artery to a transplanted kidney sustained three episodes of impaired renal function during treatment with low doses of captopril. On each occasion this was reversed by withdrawal of therapy. The effect was not related to reduction of blood pressure since lower pressures were recorded during treatment with minoxidil, without deteriorat...

Non-invasive investigations of potential renal artery stenosis in renal insufficiency.

BACKGROUND The diagnostic value of non-invasive methods for diagnosing renal artery stenosis in patients with renal insufficiency is incompletely known. METHODS Forty-seven consecutive patients with moderately impaired renal function and a clinical suspicion of renal artery stenosis were investigated with computed tomography angiography (CTA), gadolinium-enhanced magnetic resonance angiograph...
