چکیده
Martins D, et al. BMJ Case Rep 2017. doi:10.1136/bcr-2017-221360 Description This beautiful angiogram was obtained from an asymptomatic healthy 14-year-old male. Undergoing cardiovascular evaluation for sport practice, he had a normal physical examination except for a grade II low frequency murmur. Serial otherwise unremarkable ECGs revealed frequent monomorphic ventricular ectopy, which increased in frequency during an asymptomatic treadmill stress test. His echocardiogram showed a normal left ventricular systolic function with minimal anterolateral hypokinesia and an abnormal diastolic septal flow on colour Doppler. Cardiac MRI portrayed basal anterolateral myocardial fibrosis and marked ectasia of the right coronary artery (RCA), although fine coronary anatomic definition was not possible due to motion artefact. With the clinical suspicion of a coronary fistula, conventional angiography was preferred over CT for its eventual therapeutic purpose. The angiogram (figure 1, video 1) demonstrated a severely dilated RCA feeding a dense net of collaterals, which drained retrogradely into the left coronary artery (LCA) with diastolic run-off into the main pulmonary artery, diagnosing and demonstrating the pathophysiological mechanism of an anomalous LCA from pulmonary artery. This rare entity often presents as ischaemic cardiomyopathy in early infancy as the physiological drop in pulmonary vascular resistance precipitates inadequate perfusion of the LCA. However, increasing numbers of adolescent and adult cases have been described, with a more benign early course due to abundant collateral circulation, presenting later in life with exercise intolerance, ventricular dysrhythmias and sudden cardiac death due to coronary steal. 3 Surgical reimplantation Flowing back and forth: a coronary malformation Duarte Martins, Cátia Costa, Isabel Menezes, Fernando Maymone Martins Images in...

Effect of Teach- Back on Treatment Adherence in Patients with Acute Coronary Syndrome: A Semi-Experimental Study
Background and purpose: Patients with acute coronary syndrome suffer from relapse and progression of disability due to poor adherence to treatment plans. The current study aimed to determine the effect of teach-back on treatment adherence in patients with acute coronary syndrome. Materials and methods: A semi-experimental study was carried out in 70 patients with acute coronary syndrome in car...
متن کاملEmbolic pulmonary complication of a cerebral arteriovenous malformation treatment.
A 34-year-old man was admitted in the emergency room with headache, vomiting, gait ataxia, dizziness and vertigo. He was submitted to a cranial CT that showed an important hydrocephalus secondary to a forth ventricle compression by a posterior fossa hematoma. He received advanced life support in an intensive care unit (ICU) and was submitted to an external ventricular drainage (EVD), posteriorl...
متن کاملEffect of Acupressure on Low Back Pain and Fatigue after Coronary Artery Angioplasty through Femoral Artery: A Randomized Controlled Clinical Trial
Background and purpose: Complaints of low back pain and fatigue caused by prolonged lying on the back are complications of angioplasty. The aim of this study was to evaluate the effect of acupressure on severity of low back pain and fatigue in patients after coronary artery angioplasty through femoral artery. Materials and methods: A simple randomized controlled clinical trial was carried out ...
متن کاملThe Effect of Zinc Oxide Scallop-Shell Powder and Complications After Coronary Angiography
Introduction: Coronary angiography is often associated with complications such as hemorrhage and hematoma that should be considered and be minimized. Objective: This study aimed to determine the effect of Zinc oxide scallop-shell powder as a topical hemostatic agent (composed mainly of calcium) on complications after coronary angiography. These complications include hemostasis time, low back p...
متن کاملMarch 4, 2010 ISOMORPHISM TYPES OF BOOLEAN ALGEBRAS IN THE BACK-AND-FORTH HIERARCHY
We investigate the back-and-forth types which isomorphism types. The main result is that the indecomposable isomorphism types which arise at the finite levels of the back-and-forth hierarchy are the same the finitary isomorphism types in Heindorff.
