Eczema in early life: genetics, the skin barrier, and lessons learned from birth cohort studies.

چکیده
czema is a chronic inflammatory disorder of the skin that affects as much as 30% of children. It often afflicts infants in the first few months of life and can be the first indicator of the atopic march. Recent results from birth cohort studies have uncovered novel information about genetic and environmental factors that promote the development of eczema. Birth cohort studies provide an optimal study design to elucidate these associations and prospectively track longitudinal data including exposure assessment and health outcomes from birth into early life and childhood. This is especially relevant for eczema because of the age-specific emergence of this disease. In this review, we will provide a general overview of pediatric eczema and discuss the important findings in the literature for genetics and environmental exposures, highlighting those derived from birth cohort studies. Additionally, we will review how these relate to the atopic march, the hygiene hypothesis, and the integrity of the skin barrier.

Gene-Environment Interaction in the Onset of Eczema in Infancy: Filaggrin Loss-of-Function Mutations Enhanced by Neonatal Cat Exposure
BACKGROUND Loss-of-function variants in the gene encoding filaggrin (FLG) are major determinants of eczema. We hypothesized that weakening of the physical barrier in FLG-deficient individuals may potentiate the effect of environmental exposures. Therefore, we investigated whether there is an interaction between FLG loss-of-function mutations with environmental exposures (pets and dust mites) in...

BACKGROUND Prevention strategies in atopic dermatitis (AD) using allergen avoidance have not been consistently effective. New research reveals the importance of the skin barrier in the development of AD and possibly food allergy and asthma. Correcting skin barrier defects from birth may prevent AD onset or moderate disease severity. OBJECTIVE We sought to determine the feasibility of skin bar...
متن کاملThe New Zealand Asthma and Allergy Cohort Study (NZA2CS): Assembly, Demographics and Investigations
BACKGROUND Asthma and allergy are highly prevalent in industrialised countries. Longitudinal and cross-sectional studies have identified a number of potential risk factors for these conditions, including genetic and environmental factors, with significant gene-environment relationships. Birth cohort studies have been proposed as an important tool to explore these risk factors, particularly expo...

OBJECTIVE To determine if use of paracetamol in early life is an independent risk factor for childhood asthma. DESIGN Prospective birth cohort study. SETTING Melbourne Atopy Cohort Study. PARTICIPANTS 620 children with a family history of allergic disease, with paracetamol use prospectively documented on 18 occasions from birth to 2 years of age, followed until age 7 years. MAIN OUTCOME...
متن کاملPredictors of allergen sensitization in Singapore children from birth to 3 years
BACKGROUND Immune responses in allergic diseases begin with allergen sensitization, which usually occurs in childhood. Allergen sensitization involves a complex interplay of genetic and environmental factors, and sensitization patterns may change with age. OBJECTIVE To determine the predictors of allergen sensitization in the first 3 years of life in the growing up in Singapore towards health...
