چکیده

Severe constipation and hypothermia progressing to hypothermic coma are known complications of myxoedema. The occurrence of hypothermia is recognized to be an indication for urgent treatment with the most rapidly acting thyroid preparation available, l-triiodothyronine. There are only two cases of hypothermic myxoedema coma which have been successfully treated with triiodothyronine reported in the English literature (Macdonald, I958; Surtees and Ginsberg, I958), and there is no accepted regime for the dosage and mode of administration of this drug. A further case of myxoedema with hypothermia which was successfully treated is here reported, with details of the dosage of triiodothyronine used. The patient is also of interest in that she originally presented with sub-acute obstruction of the large bowel due to faecal impaction, the result of severe constipation, and at this time, four weeks before the occurrence of hypothermia, the diagnosis of myxoedema was missed.

Hypothermic Myxoedema

The photomotogram (P.M.G.) of the Achilles tendon reflex was studied in 26 patients with hypothermia (rectal temperature 333°C or less), 10 of whom also had myxoedema (serum protein bound iodine 2-8 Fg/100 ml or less). No reflex could be elicited in eight (31%) ofthese patients, including three ofthose with myxoedema. Hypothermia increases both the contraction and the relaxation times of the re...

Acute pancreatitis and diabetic ketoacidosis in accidental hypothermia and hypothermic myxoedema.

Serial serum amylase and blood glucose levels were measured in 68 hypothermic (rectal temperature 35 degrees C or less) patients, including 15 who had hypothermic myxoedema (serum protein bound iodine 3.5 mug/100 ml or less). Raised amylase levels were found in 34 patients and probably reflected a mild acute pancreatitis. The high amylase levels correlated with low arterial PO(2) levels and sig...

A case of myxoedema coma successfully treated with tri-iodo-thyronine.

Introduction The termination of myxoedema in coma was described in the Report of the Clinical Society of London(i888) and had been noted even before this (St. Thomas's Hospital Report, I879). It is an uncommon complication occurring in longstanding untreated cases and hitherto was fatal, with the possible exception of one patient (Malden, 1955), and here the cerebro-spinal fluid protein was mar...

Successful Treatment of Refractory Thrombotic Thrombocytopenic Purpura with Rituximab, a Monoclonal Antibody: a Case Report

Thrombotic thrombocytopenic purpura (TTP) is mostly attributed to the presence of an autoantibody against ADAMTS-13, a metalloprotease that degrades ultralarge von Willebrand protein multimers. Accumulation of vWF multimers and systemic platelet aggregation lead to microangiopathic thrombosis, hemolytic anemia, and end-organ ischemia. Most patients respond to therapeutic plasma exchange (TPE), ...
