Relation of variations in activation order to intraventricular pressures during premature beats.

چکیده
A comprehensive study on open chest anesthetized dogs was done to investigate the influence of intraventricular activation order on cardiac performance and to evaluate the degree to which entry into the conduction system might contribute to the synchrony of contraction in the ventricle opposite the stimulus site. Intraventricular pressures were measured during premature beats from several ventricular sites and over the entire range of the cardiac cycle. For each site, intraventricular pressures during the premature beats were plotted vs. time of delivery of the stimulus and also vs. time of onset of pressure rise in the ventricle being considered. The prior observation that higher ventricular pressures occur during premature beats initiated in the opposite ventricle was confirmed. Evidence is presented that the major factor responsible is the increased filling and recovery interval permitted by the conduction time of the activation front arriving from the opposite ventricle. After correction for filling and recovery time, there were still differences due to site of origin of the premature beat, with those delivered to the apex resulting in the highest left ventricular pressures. The hypothesis that a more synchronous contraction results from stimulation of sites on the contralateral, rather than the ipsilateral ventricle, was not substantiated. ADDITIONAL

متن کاملDoes transport affect the incidence of intraventricular hemorrhage in preterm infants?
Objective: Preterm infants are prone to intraventricular hemorrhage. They may suffer from significant morbidity and mortality particularly in those with high-grade hemorrhage. Our objective was to determine the incidence of intraventricular hemorrhage in premature infants transported to a tertiary center compared with those delivered at the level III facility. Methods: We evaluated all prematu...
متن کاملLack of sensing by demand pacemakers due to intraventricular conduction defects.
A previously unrecognized form of failure of normally functioning noncompetitive R-inhibited or R-triggered pacemakers to sense is described in 30 patients. The failure was produced by the delayed arrival of ventricular depolarization due to intraventricular conduction disturbances at the site of pacemaker sensing electrodes. These patinets with late activation of the right ventricle exhibited ...

Figure 1 A: Baseline electrocardiogram (ECG) during biventricular pacing at 60 beats per minute showing an underlying left atrial flutter and bigeminy. The premature ventricular complexes have the same morphology and a coupling interval of 630 ms, indicated by the blue rectangle. B: Baseline ECG, after reprograming the lower pacing rate at 30 beats per minute, showing poor residual atrioventric...
متن کاملAtrioventricular junctional premature and escape beats with altered QRS and fusion.
THIS STUDY presents electrocardiographic data from six patients which indicate that (1) atrioventricular (A-V) junctional premature and escape beats may give rise to QRS complexes which differ in configuration from the QRS of sinus origin and which cannot be explained by intraventricular block or aberrant conduction during incomplete recovery, (2) QRS complexes occur which seem to be produced b...
