Endoscopic Decompression, Foraminalplasty and Dorsal Rhizotomy for Foraminal Stenosis and Lumbar Spondylosis: A Hybrid Procedure in lieu of Fusion
نویسنده
چکیده
Surgical Treatment of chronic Low Back pain syndrome from an aging spine traditionally focuses on fusion as the ultimate surgical means to eliminate debilitating back pain and sciatica.” A variety of painful degenerative conditions of the lumbar spine is treated surgically by translaminar decompression and fusion. Spine surgeons trained to perform fusions, with no training in endoscopic surgery are battling an economic crisis of escalating cost and affordability when fusion surgery is recommended for chronic back pain. This is considered the ultimate treatment by surgeons favoring fusion as a means to treat perceived or actual instability and/or deformity supported by imaging

Endoscopically Guided Foraminal and Dorsal Rhizotomy for Chronic Axial Back Pain Based on Cadaver and Endoscopically Visualized Anatomic Study
BACKGROUND Conventional fluoroscopically guided continuous radiofrequency (CRF) and pulsed Radiofrequency (PRF) lesioning of the medial branch, dorsal ramus, a standard technique to treat facet pain, is compared to an endoscopic visually guided technique. The endoscopic technique (Figure 1) is designed to ablate a larger area of the transverse process where the medial branch crosses to innervat...
متن کاملTransforaminal Percutaneous Endoscopic Discectomy and Foraminoplasty after Lumbar Spinal Fusion Surgery.
BACKGROUND The most common causes of pain following lumbar spinal fusions are residual herniation, or foraminal fibrosis and foraminal stenosis that is ignored, untreated, or undertreated. The original surgeon may advise his patient that nothing more can be done in his opinion that the nerve was visually decompressed by the original surgery. Post-operative imaging or electrophysiological assess...
متن کاملIn-vivo Endoscopic Visualization of Pain Generators in the Lumbar Spine
Introduction: Traditional interventional pain management only provides temporary relief that depend on the patient’s natural healing to mitigate pain. Visualizing the patho-anatomy with an endoscope targeting the pathoanatomy by interventional needle trajectories, however, has opened the door for surgical decompression and ablation of the pain generators. Endoscopic spine surgery is effective u...
متن کاملPercutaneous Endoscopic Interlaminar Approach: Medial Foraminal Decompression in Treating Lumbar Disc Herniation or Spinal Stenosis
Background: The technique of transforaminal endoscopic spine surgery is being widely used for lumbar degenerative diseases. But the interlaminar endoscopic surgery, which is more familiar and easier to be operated for spine surgeons, is more easily applied by traditionally trained surgeons. Objective: We propose the technique of percutaneous endoscopic medial foraminal decompression through int...
متن کاملRadiographic Assessment on Magnetic Resonance Imaging after Percutaneous Endoscopic Lumbar Foraminotomy
Percutaneous endoscopic lumbar foraminotomy (ELF) is a novel minimally invasive technique used to treat lumbar foraminal stenosis. However, the validity of foraminal decompression based on quantitative assessment using magnetic resonance imaging (MRI) has not yet been established. The objective of this study was to investigate the radiographic efficiency of ELF using MRI. Radiographic changes o...
