بررسی مقایسه‌ای شیوع اعتیاد به تریاک در انفارکتوس میوکاردخاموش (Silent) با غیرخاموش (Non-Silent)

مقدمه: انفارکتوس میوکارد خاموش نوعی انفارکتوس قلبی است که در زمان بروز تشخیص داده نمی شود و تنها بطور تصادفی مشخص می‌گردد. هدف: این بررسی برای تعیین شیوع اعتیاد به تریاک در انفارکتوس میوکارد خاموش و مقایسه آن باانفارکتوس میوکارد غیرخاموش انجام گردید مواد و روش‌ها: 676 بیمار (495 نفرمذکر، 181 نفر مؤنث) که برمبنای الکتروکاردیوگرام با تشخیص انفارکتوس میوکارد تمام دیواره‌ای به یکی ازکلینیک‌های سرپائی خصوصی شهرستان رشت مراجعه کرده بودند مورد بررسی قرارگرفتند. موارد انفارکتوس میوکارد زیر دیواره‌ای و کاذب حذف شد. نتایج: 231 نفر (2/34درصد) از بیماران، هیچگونه سابقه و شرح حال ویژه انفارکتوس‌میوکارد را نداشتند(انفارکتوس خاموش). شیوع کشیدن تریاک در انفارکتوس میوکارد غیرخاموش بطور بارزی از انفارکتوس‌میوکارد خاموش بیشتر بود (5/17 در برابر 5/6 درصد، P کمتر از 001/0). شیوع خوردن تریاک در انفارکتوس میوکارد خاموش (3/4 درصد) تفاوت بارزی ازنظر آماری با انفارکتوس غیرخاموش (9/2 درصد) نداشت. شیوع مصرف تریاک (کشیدن و خوردن ) در انفارکتوس غیرخاموش بطور بارزی ازنوع خاموش بیشتر بود (4/20 در برابر 8/10 درصد، P کمتراز 005/0). میانگین سنی (P کمتر از 0001/0 )، جنس مؤنث (P کمتر از 0001/0)، افزایش‌فشارخون شریانی در مردان (P کمتر از 0001/0)، دیابت قندی (P کمتر از 05/0)، وسکته مغزی در مردان (P کمتر از 005/0 ) بطور بارزی در انفارکتوس خاموش بیشتر وشایعتر از نوع غیرخاموش بود. نتیجه گیری: این بررسی نشان می‌دهد که : 1 - اعتیاد به تریاک از عوامل همراه انفارکتوس میوکاردخاموش نمی‌باشد. 2 - عواملی چون سن بالا، جنس مونث، دیابت قندی، و افزایش‌فشارخون شریانی و سکته مغزی (در مردان)، از عوامل همراه انفارکتوس میوکاردخاموش می‌باشند.
