مطالعه یافته های سونوگرافی در عفونتهای ادراری علامت دار در کودکان بستری در بیمارستان بهرامی

Abstract:
عفونت ادراری از شایعترین عفونتهای دوران کودکی است که هنوز مرگ و میر و موربیدیتی قابل توجهی دارد. بنا به توصیه بسیاری از مراجع پس از تشخیص آن، در همه کودکان مبتلا سونوگرافی انجام می شود. در حالیکه بعضی دیگر، انجام سونوگرافی را محدود به موارد خاصی می دانند. هدف این مطالعه توصیفی گذشته نگر بررسی نتایج سونوگرافی در کودکان مبتلا به عفونت ادراری بوده است. در این راستا بیمارانی که طی 10 سال (بهمن 1367 تا بهمن 1377) در بیمارستان بهرامی با تشخیص UTI بستری شدند مورد بررسی قرار گرفت. در همه این کودکان سونوگرافی سیستم ادراری توسط یک رادیولوژیست مجرب انجام شده است. 148 مورد عفونت ادراری بررسی شد که از این تعداد 125 مورد UTI توسط کشت اثبات شده و سونوگرافی انجام شده است. 78 درصد بیماران دختر و 22 درصد پسر بودند.شیوع UTI با افزایش سن کاهش یافته و شایعترین گروه سنی 9-6سال بود. 91.2 درصد بیماران در هنگام مراجعه تب داشتند و شایعترین سن تب 5-2 سال بود. با افزایش سن شیوع تب کاهش می یافت. شایعترین عامل بیماریزا ای کلی بوده است. در 35 درصد بیماران سونوگرافی طبیعی بوده و علایم عفونت ادراری در 35 درصد و آنومالی همراه در 24 درصد وجود داشت. شایعترین یافته عفونت افزایش ضخامت جدار مثانه و شایعترین اختلال همراه هیدرونفروز بوده است.
