Abstract:

Background: Carpal tunnel syndrome is the most common focal mono-neuropathy. A study was designed to compare the effects of traditional open carpal tunnel release with median neurolysis and Z-plasty reconstruction of the transverse carpal ligament on post-operative hand pain and hand function in patients with idiopathic carpal tunnel syndrome.   Methods: Fifty-two patients with idiopathic carpal tunnel syndrome entered the study. The patients were randomly assigned into two groups to undergo simple transverse carpal ligament release or division of the ligament with Z-lengthening reconstruction. Forty-five patients completed the study. Two patients of the simple open surgery group and 5 patients of the Z-plasty reconstruction group did not complete the follow up course. After the procedure, the patients were followed to assess post-operative pain and hand function during a 12-week period.   Results: The scores of hand pain on the first day after surgery were not statistically different between the two groups (P=0.213). But the score of hand pain was significantly lower in the Z-plasty reconstruction group at week 1, week 3, and week 6 after surgery (P

Noninvasive laser neurolysis in carpal tunnel syndrome.

The peripheral nervous system is photosensitive providing the scientific rationale for this study, which determines the efficacy and safety of laser light exposure in 30 cases with CTS. Nine joules of energy over five points (7-15 treatments) reversed CTS in 77% of cases with three-fold normalization of CMAP. A photobiologic response was seen in 80% of nerves. This unique and novel approach is ...

Inflammation and fibrosis of transverse carpal ligament and flexor tenosynovium in severe idiopathic carpal tunnel syndrome

Background: We approached a histopathologic study of idiopathic carpal tunnel syndrome (CTS) to identify its probable causative factors. The purpose of this study was to assess the prevalence of inflammation and fibrosis in the transverse carpal ligament (TCL) and flexor tenosynovium (FT) in severe idiopathic carpal tunnel syndrome. Methods: Thirty nine patients with severe idiopathic CTS under...

Effect of Rehabilitation Interventions on Sensation, Function, and Pain of the Patients With Carpal Tunnel Release

Objectives: Carpal Tunnel Syndrome (CTS) is now the most common type of nerve compression neuropathies constituting about (90%) of the neuropathic disorders. To attain the optimal recovery following the operation, a rehabilitation intervention is urgent. This study aims at investigating the efficacy of short and long term rehabilitation interventions following Carpal Tunnel Release (CTR) on mot...
