Abstract:

ABSTRACT Cardiac glycosides have been used in the management of heart failure for more than 200 years and remain the only drug currently available for Long-term ambulatory use that have positive inotropic effect. The relatively narrow therapeutic- index of cardiac glycosides renders them particularly difficult to use, and the clinician should have a detailed understanding of the actions and pharmacokinetics of one drug of this class, such as digoxin. The Purpose of this study was determination of frequency of abnormal digoxin level in patients who was taking this drug. We studied SDC(Serum Digoxin Concentration) of Patients referred to nuclear medicine department of Dr. Heshmat hospital inpatients or outpatients status from 1995 to 1999. Digoxin serum concentration were measured using radioimmunoassay technique. Data were analyzed by using chi-square test. 668 (69.5%) of 960 Patients had SDC out of therapeutic range. Elevated SDC(>2ng/ml) occurred in 202 (21%) Patients, but 466 (48.5%) had SDC below therapeutic range (<1.1 ng/ml). There was not significant difference between sex and age with SDC (P>0.05). Comparison with Pervious studies, our research showed much more abnormal SDCs.

Quinidine-digoxin interaction: cardiac efficacy of elevated serum digoxin concentration.

Cardioactivity due to elevated serum digoxin concentration (SDC) after quinidine (Q) and digoxin (D) was evaluated in six healthy subjects by means of measurement of systolic time intervals (STIs). Each subject randomly received basic treatments with 0.2 mg D and placebo (PL1). Randomized coadministrations with Q (1 gm/day), sparteine (SP) (0.8 gm/day), and placebo (PL2) were given for 7-day pe...

Assessment of the appropriateness of serum digoxin concentration measurement in a medical group setting.

BACKGROUND Recent quality initiatives require that the routine annual therapeutic drug-monitoring (TDM) parameters for the high-risk medication digoxin include a measure of renal function and a serum potassium level but not a serum digoxin concentration (SDC) measurement. Several studies have shown that the majority of the SDCs obtained in hospital settings provide little clinically actionable ...

The influence of verapamil on serum digoxin concentration.

The effect of verapamil on the pharmacokinetics of digoxin was studied in 49 patients with chronic atrial fibrillation. A dose of 240 mg/day of verapamil was given to the patients who were receiving a stable dose of digoxin. Serum digoxin levels rose from 0.76 +/- 0.54 ng/ml (mean +/- SD) to 1.31 +/- 0.54 ng/ml during verapamil treatment (p less than 0.0005). This effect was dose-dependent, as ...

Serum cystatin C levels to predict serum concentration of digoxin in Japanese patients

Cystatin C (Cys-C) has been recently paid great attention as a better endogenous marker of the glomerular filtration rate than creatinine (Cr). In this study, the usefulness of Cys-C was compared with Cr in terms of the estimation of the steady-state serum trough concentrations of digoxin in Japanese patients. Forty patients treated with digoxin and 56 healthy elderly subjects were participated...

Serum copper concentration significantly less in abnormal pregnancies.

We estimated copper concentration in maternal serum during 244 normal and 15 abnormal pregnancies. Values were lower in the abnormal pregnancies than in the normal ones, and did not vary with gestational age between 15 to 18 weeks in normal pregnancies.

Radioimmunoassay of serum digoxin in relation to digoxin intoxication.

Serum digoxin estimations were done in 98 patients receiving digoxin for heart failure of varied aetiology. Digoxin toxicity or the lack of it was determined on the basin of established electrocardiographic criteria. Fifty-two patients were classified as 'toxic' and 46 as 'non-toxic'. The difference is the mean digoxin levels between the two groups was highly significant (P less than 0.001). Th...
