Cutaneous Hyalohyphomycosis in a Woman with Normal Immune System

Abstract:

Paecilomyces Sp. are saprophytic fungi, which have rarely been pathogen for human. Herein, we report a case of cutaneous infection with Paecilomyces lilacinus with unusual presentations in a healthy young woman. A biopsy provided an initial diagnosis of fungal elements in the tissue. Multiple positive fungal cultures were obtained from the biopsied tissue. Microscopic and macroscopic examination of the biopsy revealed the presence of Paecilomyces lilacinus. This case was successfully treated by prescribing oral ketoconazole (200 mg/day).

Localized cutaneous hyalohyphomycosis caused by a Fusarium species infection in a renal transplant patient.

Fusariosis is a hyalohyphomycosis due to Fusarium species that mainly occurs in immunocompromised hosts. The clinical spectrum of Fusarium infection comprises localized and disseminated forms. A case of localized cutaneous fusariosis caused by Fusarium solani in a renal transplant patient is described, and the skin manifestations of the disease are discussed.

A case of rapidly progressive cutaneous infection due to Paecilomyces lilacinus developed in a woman with advanced pancreatic cancer who did not have granulocytopenia. The infection responded favorably to caspofungin and itraconazole combination therapy.
