چکیده مقاله:

Background and Objective: The injection site in spinal anesthesia is one of the factors that can affect the height of the sensory block. This clinical trial study aimed to compare the effect of spinal anesthesia at L3-L4 and L4-L5 on the level of sensory-motor block and hemodynamic status in cesarean section. Materials and Methods: Seventy patients undergoing elective cesarean section and receiving spinal anesthesia (10 mg of 0.5% bupivacaine plus 2.5 μg of sufentanil) were randomly divided into two groups of 35 with spinal anesthesia at L3-L4 (group 1) or L4-L5 (group 2). The two groups were compared concerning demographic data, hemodynamic status, level of sensory block, degree of motor block, time to return the sensory level to T10, complications of spinal anesthesia, and neonate Apgar score. Results: There was no significant difference between the two groups in the mean age (P=0.329). In the first group, 22.9%, and in the second group, 40% of the patients had the level of anesthesia at the eighth thoracic vertebra (T8). None of the patients had zero or one score in both groups. Hypotension (82.9% vs. 8.6%), bradycardia (34.3% vs. 8.6%), and intraoperative nausea and vomiting (65.7% vs. 11.4%) were higher in the first group than in the second group (P<0.001). Conclusion: Performing spinal anesthesia in L4-L5 lumbar space, compared to L3-L4 space, in addition to creating a sufficient level of anesthesia, is associated with fewer complications, such as hypotension, bradycardia, as well as nausea and vomiting, in cesarean section.

A comparison of outcomes between L3/L4 and L4/ L5 single-level laminectomy surgery

Background: Decompressive laminectomy is the most common operation performed to treat spinal stenosis. This study was performed in order to evaluate surgical outcomes between laminectomy at the L3/L4 level compared with L4/ L5. Methods: The patients diagnosed with one level stenosis at L3/L4 or L4/ L5 who were candidate for surgery entered into this cross-sectional study. The outcome measures w...

COMPARISON OF HEMODYNAMIC CHANGES DUE TO INTRAVENOUS OXYTOCIN INFUSION DURING GENERAL AND SPINAL ANESTHESIA IN ELECTIVE CESAREAN SECTION

Background & Aims: Postpartum hemorrhage is one of the life-threatening complications after labor and cesarean section. Its prevalence is 6 percent. Metratonia is the most common cause of this complication. Some medications can prevent it. Oxytocin is the most common uterine vasoconstrictor and is used routinely after delivery for decreasing hemorrhage. The aim of this study is to compare hemod...

L3-L4 dislocation without neurological lesions.

Vertebral dislocations are high energy injuries that rarely occur in the low back, but are found more frequently at the level of the thoracolumbar and sacrolumbar junctions. Dislocations of the mid-lumbar vertebrae are exceptional, with only 16 cases found in the literature. All previously reported cases were treated surgically. In four of these cases, the patients had no initial neurological l...

Surgical Experience in Cases of L5 and S1 Symptoms Caused by Upper Lumbar Spinal Stenosis of L2–L3 and L3–L4

Object: Previous reports have indicated that the level of lumbar spinal canal stenosis (LSCS) often differs from that diagnosed from neurological symptoms, and L5 nerve roots are often affected by stenosis at the L2–L3 or L3–L4 level; however, few cases have been described. Decompression surgery for upper lumbar spinal canal stenosis (LSCS) of L2–L3 and L3–L4 causing L5 and S1 symptoms was inve...

Right foraminal disk protrusion at L3-L4.

Vincent M. Arlet, MD, and Andrew H. Milby, MD: The provided history, examination findings, and imaging studies are all consistent with the diagnosis of acute radiculopathy due to lumbar disk protrusion. Pain localized to the anterior thigh in combination with quadriceps weakness suggests involvement of the ipsilateral L3 or L4 nerve roots. Relief of pain with hyperflexion is a reverse-tension s...
