چکیده مقاله:

Background: Community-acquired infections by multidrug-resistant (MDR), extended-spectrum β-lactamase (ESBL) producing Klebsiella species (Klebsiella spp.) is of major concern worldwide. Antibiotic resistance, production of extended-spectrum β-lactamases (ESBLs), and carbapenemases, as well as the presence of classes 1, 2, and 3 integrons in outpatient isolates of Klebsiella collected from Yazd central laboratory, Yazd, Iran. Methods: We collected 250 Klebsiella isolates from Yazd central laboratory between August 2015 and October 2017. Antibiotic susceptibility was determined against 18 antibiotics by disc diffusion, and multidrug-resistant isolates were tested for ESBL production by the phenotypic confirmatory test according to CLSI 2017 protocols. The amplification of β-lactamase genes blaSHV, blaTEM, blaCTX-M, blaOXA-48, blaKPC, and blaNDM, classes 1, 2, and 3 integrase genes, was carried out using specific primers and polymerase chain reaction (PCR).Results: Of the 250 Klebsiella outpatient isolates, 3.6% were K. oxytoca and the rest were K. pneumoniae. Disc diffusion showed that 21 (8.4%) isolates were MDR, 19 (90.4%) of which were ESBL producers including one K. oxytoca. The most prevalent β-lactamase gene was blaSHV followed by blaTEM and blaCTX-M, but blaOXA-48, blaKPC, and blaNDM were not detected. Class 1 integron was detected in 18 out of 21 MDR isolates (85.7%), but classes 2 and 3 were not observed. Two isolates were resistant to carbapenems and harbored blaSHV, blaTEM, and blaCTX-M, as well as class 1 integron. Conclusion: ESBL production and the presence of multiple β-lactamase genes in MDR community isolates of Klebsiella spp. can have significant implications in terms of the spread of these opportunistic pathogens.

Prevalence of qnr Genes in Extended-Spectrum β-Lactamase Producing Klebsiella pneumoniae Isolated from Clinical Urine Specimens in University Teaching Hospitals, Iran

Background and Aims: Extended-spectrum β-lactamase (ESBL) producing gram negative bacteria are resistant to penicillins, narrow and extended-spectrum cephalosporins, and aztreonam; also they are frequently resistant to trimethoprim-sulfamethoxazole, aminoglycosides, and quinolones. This study aimed to investigate the prevalence of plasmid-mediated quinolone resistance (PMQR) determinants q...

Correlation of quinolone-resistance, qnr genes and integron carriage in multidrug-resistant community isolates of Klebsiella spp.

Objective(s): Plasmid-mediated quinolone resistance (PMQR) determinants and integrons have a considerable contribution to bacterial drug resistance in Gram-negative pathogens. We studied the prevalence of PMQR genes and integron carriage in multidrug-resistant community isolates of Klebsiella spp.Materials and Methods: Two hundred and fi...

Integron mediated multidrug resistance in extended spectrum beta-lactamase producing clinical isolates of Klebsiella pneumoniae

The present study describes integron mediated multiple antibiotic resistance in extended-spectrum β-lactamase producing clinical isolates of Klebsiella pneumoniae. One hundred and four clinical isolates of K. pneumoniae from two Iranian hospitals were screened for extended-spectrum β-lactamase production and susceptibility of the extended-spectrum β-lactamase producing isolates was determined t...

Prevalence of extended spectrum β-lactamase (ESBL) producing E. coli and Klebsiella species in urinary isolates

Introduction: Bacterial resistance to β-lactam antibiotics has risen dramatically, with extended spectrum βlactamase (ESBL) contributing to this increase. Early identification of ESBL production is becoming increasingly important. Methodology: A total of 367 consecutive, urine samples from patients suspected to have UTI, were processed. All Escherichia coli and Klebsiella species isolated in si...

Prevalence of blaCTX-M Gene among Extended- Spectrum β-Lactamases Producing Klebsiella pneumoniae Clinical Isolates in Iran: A Meta-Analysis

Background: CTX-M-type extended-spectrum β-lactamases (ESBLs) are the most prevalent ESBLs in bacterial members of Enterobacteriaceae family including Klebsiella pneumoniae. The global spread of CTX-M-producing K. pneumoniae is a major concern in most countries including Iran. The aim of this meta-analysis was to determine the relative frequency (RF) of blaCTX-M gene among ESBLs-producing K. pn...

Extended – Spectrum β-Lactamase Producing Klebsiella Species among Clinical Isolates

Extended – Spectrum Βeta-Lactamase producing strains among clinical isolates has been steadily increasing over the past years. Microorganisms responsible for UTIs such as Escherichia coli and klebsiella 5pp have the ability to produce ESBLs in large quantities. All the strains of klebsiella species in this study were collected from routine clinical samples confirmed by biochemical and other tes...
