【背景】HLA抗体による血小板輸血不応症例には,HLA適合血小板(PC-HLA)が推奨される.ドナーの選定には,HLA-A座,B座の適合性を確認するが,HLA-C座の適合性は優先されない.本研究では,HLA-C座の適合性による輸血効果を解析した.【方法】関東甲信越ブロックにおいてHLA-C抗体による交差適合試験陽性例の輸血効果を解析した.また,東京大学医学部附属病院においてHLA抗体を保有する血小板輸血不応患者1例に対してPC-HLA(66本),適合血小板(19本)およびランダムの血小板製剤(7本)について,輸血効果を評価し,後追いの交差適合試験を実施した.【結果】交差適合試験陽性57件(陽性率0.25%)のうちHLA-C抗体によるものは27件(47.4%)であり,Cw8抗体が55.0%を占めていた.HLA-C座不一致製剤の75%は有効であった.東大01症例はCw15抗体陽性かつCw...
